腹腔镜筋膜内子宫切除术对近期女性生活质量的影响

目的:评价腹腔镜筋膜内子宫切除术(ClassicalIntrafascialSupracervicalHysterectomy,CISH)对女性近期生活质量的影响,探讨子宫切除的最佳术式。方法:选择49岁以下因良性疾病需行单纯子宫切除的非绝经妇女43例(CISH组),并选择同期在该院行单纯开腹子宫切除术且条件相似的患者(AH组)43例作为对照。比较2组的手术时间、术中出血量、术后排气时间、术后病率等情况。并于术后6个月对阴道长度、术后性生活及大小便情况进行随访。结果:CISH组患者术后性生活满意度优于AH组,CISH术对患者膀胱功能无不良影响,对直肠功能影响较小,与AH组相比差异有显著性(P<0.05);且CISH组术中出血少、术后排气时间短、术后病率低(P<0.05)。结论:CISH对女性近期生活质量影响不明显,其治疗效果优于AH,值得在临床推广应用。

Effect of classical intrafascial supracervical hysterectomy on short-term quality of life of women

Objective:To value the effect of classical intrafascial supracervical hysterectomy( CISH )on quality of life in women, and to discuss the best operation for hysterectomy.Methods:43 non-menopausal women under 49-year-old who suffered from gynaecologic benign diseases and had hysterectomy signs were chosen as CISH group.43 women who had simlar condition at the same time were selected as AH group.Operation time, bleed amount in operation ,postoperative exhausting time and postoperative morbidity were compared between the two groups.The length of vagina, sexual function ,urinary and bowel function were investigated 6 months after operation.Results:The sexual life satisfaction was higher markedly in CISH group than that in AH group.CISH caused less effects on bladder function and bowel function.There were significant differences between two groups(P<0.05).Compared with AH group, CISH group had less operation bleeding, shorter postoperative exhausting time, lower febrile morbidity (P<0.05).Conclusion:CISH's effect on quality of life in women was not significant. CISH has significantly better effect than AH, and is good for use in clinic.
