无创正压通气治疗COPD并呼吸衰竭疗效观察

摘    要:耳的评价无创正压通气(NPPV)治疗慢性阻塞性肺疾病(COPD)呼吸衰竭临床应用价值。方法回顾性分析COPD并呼吸衰竭86例患者的临床资料,比较使用NPPV和未使用NPPV治疗前后动脉血气分析及临床症状。结果采用NPPV治疗1d后患者pH值升高(P〈0.05),PaO2明显升高(P〈0.01),PaCO2明显降低(P〈0.01),未采用NPPV治疗1d后PaO2升高(P〈0.05),但pH值仍无显著改善(P〉0.05),同时,NPPV治疗组患者心脑并发症和死亡比例明显低于非NPPV组(P〈0.01)。结论NPPV治疗COPD并呼吸衰竭在早期可纠正缺氧和CO2潴留,减少心脑并发症,降低死亡率。

Clinical observation on noninvasive positive pressure ventilaifim in the treatment of COPD with respiratory failure

Abstract:Objective To evaluate the non - invasive positive pressure ventilation (NPPV) in the treatment of chronic obstructive pulmonary'disease (COPD) with respiratory failure. Methods A retrospective analysis of COPD with respiratory failure on 86 cases, compared with the befor and after treatment NPPV arterial blood gas analysis and clinical symptoms. Results NPPV days after a treatment of patients with pH value increased ( P 〈 0.05 ), PaO2 significantly higher ( P 〈 0. 01 ), PaCO2 pressure decreased significantly ( P 〈 0.01 ), did not use NPPV days after a treatment increased PaO2 ( P 〈 0. 05 ), but the PH value of no significant improvement (P 〉 0.05 ), at the same time, NPPV in the treatment group and complications in patients with cardiovascular and cerebrovascular death was significantly lower than the proportion of non - NPPV group ( P 〈 0.01 ). Conclusion NPPV respiratory failure and treatment of COPD in the early and correct hypoxia CO2 retention and reduce cardiovascular and eerebrovascular complications and lower mortality.
