额部肌皮瓣在鼻部缺损修复中的应用

目的　探讨额部肌皮瓣在鼻部缺损中的应用。方法　应用带血管蒂的侧额瓣或额正中肌皮瓣修复 7例各种原因引起的鼻部缺损。结果　用半侧额瓣修复 6例 ,Ⅰ期愈合 4例 ,肌皮瓣部分表皮坏死 1例 ,全部表皮坏死 1例 ;用额正中肌皮瓣修复全鼻缺损 1例 ,Ⅰ期愈合。结论　额部肌皮瓣的血运丰富 ,使用面积大 ,易于成形 ,适合于各种原因引起的鼻部缺损的修复。带血管蒂的额瓣如需要经过鼻根隧道时 ,其宽松度要适当 ,过紧时易出现肌皮瓣缺血或动脉血供过于充足而静脉回流受阻产生瘀血 ,影响皮瓣成活

Experience of using pedicel frontal flap in repairing of nasal defects

Objective To evaluate the usage of pedicel frontal flap in reconstruction of nasal defects. Methods 7 cases with nasal defects (6 with partial defects and 1 with all nasal defects) were treated by using lateral (6 cases) or median (1 case) vascular pedicel frontal flap. Results Among 6 patients who were treated with lateral forehead flap, complete success was obtained in 4, partial necrosis occurred in one, complete failure occurred in one. Another one with median frontal flap was complete success. Conclusions Pedicel frontal flap is a pliable, well blood supply soft tissues flap; it is suitable for repairing nose defects. The tunnel through which the flap was transferred to the defect should be wide enough to avoid flap necrosis because of pedicel flap constriction.
