2型糖尿病患者尿白蛋白排泄率及高尿酸血症与颈动脉内膜中层厚度的关系

目的:探讨2型糖尿病患者尿白蛋白排泄率及高尿酸血症与大血管动脉粥样硬化关系。方法:用高分辨率超声对137例2型糖尿病患者的颈动脉内膜厚度(IMT)进行检测,同时测定尿蛋白排泄率(AER)、尿酸及其他常规生化指标,分析颈动脉IMT与AER、高尿酸血症及其他常规血生化指标的相关性。结果:(1)2型糖尿病患者AER与糖尿病病程、高血压史、空腹血糖(FBG)及血肌酐有关,与颈动脉IMT增厚独立相关;(2)高尿酸血症不是颈动脉IMT增厚独立相关的因素;(3)Logistic多元逐步回归分析显示AER、年龄和高血压病史与颈动脉IMT增厚独立相关。结论:尿蛋白排泄率增加是2型糖尿病患者大血管动脉粥样硬化的独立相关因素,高尿酸血症不是其独立相关因素。

Relationship of urimary albumin excretion ratio and hyperuricemla with carotid arterial intima-media thickness in patients with type 2 diabetes

Objective To investigate the relationship of urinary albumin excretion ratio(AER) and hyperuricemia to macrovascular atherosclerosis in type 2 diabetic patients.Methods Carotid arterial intima-media thickness(IMT)was measured in 137 patients with type 2 diabetes by high resolution doppler ultrasonography and urinary AER and uric acid were also detected.The relationship of urinary AER and hyperuricemia with carotid arterial IMT was statistically analyzed.Results The urinary AER was related to course of the disease,a history of hypertension,fast blood glucose,and serum creatine.Carotid arterial IMT was independently correlated with AER but not with hyperuricemia.Logistic multiple regression revealed that carotid arterial IMT was independently associated with AER,age,and a history of hypertension.Conclusion The elevated urinary AER but not hyperuricemia is an independent risk factor for macrovascular atherosclerosis in type 2 diabetic patients.
