全数字化乳腺机X线立体定位留置导丝穿刺活检术诊断乳腺微小病变的临床有效性研究

摘    要:目的 探讨全数字化乳腺机X线立体定位留置导丝穿刺活检术(full field digital mammography stereotactic core needle biopsy,FFDM SCNB)诊断乳腺微小病变(nonpalpable breast lesion,NPBL)的临床有效性。方法 将笔者医院2011年10月~2013年10月期间收治61例NPBL患者作为观察对象,分别给予FFDM SCNB和病理检验(histopathological examination,HE),诊断后按标准进行诊断准确率评定,并对数据进行统计学分析。结果 SCNB共检出NPBL 82个,其中恶性NPBL 40(48.78%)个,良性NPBL 40(48.78%)个,良恶性不明NPBL 2(2.22%)个;HE共检出恶性NPBL 40(48.78%)个,良性NPBL 42(51.22%)个,良恶性不明 0(0.00%)个;SCNB诊断准确74(90.24%)例,误诊6(7.32%)例,漏诊2(2.44%)例,与HE诊断结果相比较差异无统计学意义(P>0.05)。结论 本次研究通过对61例NPBL患者的对比诊断,发现FFDM SCNB在患者的诊断准确率方面与HE差异无统计学意义(P>0.05),且有创伤小、破坏性小、无瘢痕和经济安全等优势,值得推广借鉴。

Abstract:Objective To investigate the full digital mammography X-ray stereotactic biopsy catheter guide wire (full field digital mammography stereotactic core needle biopsy, FFDM SCNB) diagnosis of small breast lesions (nonpalpable breast lesion, NPBL) clinical effectiveness. Methods Admitted in our hospital from October 2011 to October 2013 period 61 cases NPBL patients as observed objects were given FFDM SCNB and pathological examination (histopathological examination, HE), after the diagnostic accuracy of the standard diagnostic evaluation, and data were statistically analyzed. Results The SCNB NPBL 82 were detected, of which malignant NPBL 40 (48.78%), benign NPBL 40 (48.78%), benign and malignant unknown NPBL 2 (2.22%); HE were detected in malignant NPBL 40 (48.78%), benign NPBL 42 (51.22%), benign and malignant unknown 0 (0.00%); SCNB diagnostic accuracy of 74 (90.24%) cases were misdiagnosed 6 (7.32%) cases were missed 2 (2.44%) patients, compared with no diagnosis HE significant difference(P> 0.05). Conclusion This study was to compare the diagnosis of 61 cases of patients NPBL found FFDM SCNB not in terms of diagnostic accuracy in patients with HE significant difference(P> 0.05), and there is less invasive, small-destructive, no scars, and economic security advantage, worthy reference.
