骨原发性恶性淋巴瘤组织病理学、免疫组化及形态定量研究

目的　本文对 7例骨原发性恶性淋巴瘤 (PrimaryLymphomaofBone ,PLB)进行免疫组化及病理形态定量测定研究。方法　对瘤组织采用常规石蜡切片、HE染色及ABC法免疫组化染色 ,光镜观察。应用武汉同济医科大学千屏影像公司HPIAS - 10 0 0彩色图像分析系统对肿瘤细胞核的形态进行定量测定。结果　 7例中B小淋巴细胞型 2例 ,小裂核型 2例 ,大无裂型 1例 ,多形T型 1例 ,组织细胞型 1例。形态定量测定结果表明 5种类型肿瘤细胞核的 15项形态学参数与对照组之间具有高度显著性 (P <0 .0 0 1)或显著性差异 (P <0 .0 5)。各型淋巴瘤细胞核的 15项形态学参数之间亦大多数具有高度显著性 (P <0 .0 0 1)或显著性差异 (P <0 .0 5)。结论　形态定量测定的方法对于PLB的正确诊断具有重要的参考价值 ,并对PLB的组织学分型能够提供客观的、准确的定量依据 ,因而具有十分重要的意义。其对于恶性淋巴瘤患者预后的判断亦可能具有重要意义。

The Immunohistochemistry and Quantitative Morphology Study of Primary Lymphoma of Bone

Objective To investigate the characteristics of immunohistochemistry stain and quantitative morphology measurement for primary lymphoma of Bone (PLB) in 7 cases. Methods The routine paraffin slides were stained with H.E and immunochemical ABC methods. Then,they were observed under optical microscopy. The lymphoma cell nuclei were measured with the HPIAS-1000 color pathology picture analysis system. Results Of the 7 cases, 2 was B small lymphocytic lymphoma, 2 small cleaved cell lymphoma, 1 large no cleaved cell lymphoma, 1 pleomorphic T cell lymphoma, 1 histiocytic lymphoma. The morphology measurement results showed that there was high remarkable significance (P<0.001) or remarkable (P<0.05) significance for most of the 15 items of morphologic parameters between the 5 types of lymphoma and the normal control (except the elliptical degree between pleomorphic T cell lymphoma and normal control). There was also high remarkable significance (P<0.001) or remarkable significance (P<0.05) for most of the 15 items of morphologic parameters among the different types of lymphomas. Conclusions The method of quantitative morphology measurement was very valuable for correct diagnosis, and sub classification of PLB. So that it was great significance. It may also have significance in prediction of the patient's prognosis.
