米非司酮在血液、蜕膜和绒毛中的浓度及其对流产结局的影响

本文探讨了米非司酮在血液、蜕膜和绒毛中的浓度及其对流产结局的影响。服米索后６ｈ内（４～６ｈ）胎囊排出者为Ⅰ组，共３８例；服米索后６ｈ胎囊仍未排出即刻刮宫者为Ⅱ组，共２０例。应用ＨＰＬＣ分析法检测Ⅰ组、Ⅱ组早孕妇女口服米非司酮后该药物在血液、蜕膜和绒毛组织中的浓度。结果Ⅰ组用药后不同时间血液米非司酮浓度均高于Ⅱ组（Ｐ＜０．０１）；两组间绒毛组织中药物浓度无显著性差异（Ｐ＞０．０５）。蜕膜组织中，Ⅱ组药物浓度低于Ⅰ组；两组绒毛组织中米非司酮浓度均高于蜕膜组织（Ｐ＜０．０１）。实验结果提示米非司酮不但作用于蜕膜还可直接作用于绒毛组织，对流产结局产生一定影响。

The Effect of Concentration of Mifepristone in Blood, Decidua and Villus on Outcome of Terminating Early Pregnancy

Objective To study the effect of mifepristone on the outcome of terminating early pregnan- cy. Method Group Ⅰ:Embryonic cysts were expelled less than 6 h. Group Ⅱ : Embryonic cysts were not expelled less than 6 h and were discharged by curettaged. The distributions of mifepris- tone in blood, decidua and villus were determined by HPLC from 38 cases of test group Ⅰ and 20 cases of test group Ⅱ . Results The blood concentrations of mifepristone at various time after oral ingestion in group Ⅰ were higher than those in group Ⅱ (P<0. 01). There were no signifi- cant difference in villus between two groups (P>0. 05), but the medicine levels in decidua tended to be lower in the group Ⅱ . In two groups, the medicine concentrations in villus were significant- ly higher than those of decidua(P<0. 01). Conclusion The blood levels of mifepristone in group Ⅰwere higher than those of group Ⅱ at various time after oral ingestion. It shows that mifepris- tone may have an influence on villus directly and, in some extend, mifepristone affects on the outcome of terminating early pregnancy.
