肝脏调节急性出血坏死性胰腺炎患者血清IL-6水平的作用

目的探讨肝脏在急性出血坏死性胰腺炎中所起的作用.方法将Wistar大鼠分为对照组、门腔静脉分流组、急性出血坏死性胰腺炎组、门腔静脉分流后急性出血坏死性胰腺炎组,观察各组生存时间.同样再分4组,3h后,测定每组动物血清和(或)腹水中IL-6水平.结果门腔静脉分流后急性出血坏死性胰腺炎组存活时间明显减少,与其他各组比较,差异有显著意义(P＜0.01),其血清IL-6水平明显高于其他各组,差异有显著意义(P＜0.05).急性出血坏死性胰腺炎组腹水中IL-6水平明显高于血清IL-6水平(P＜0.01),且高于门腔静脉分流后急性出血坏死性胰腺炎组腹水中IL-6水平(P＜0.05).结论肝脏在急性出血坏死性胰腺炎的早期起保护作用,其机制可能是对门静脉系统的毒性物质起屏障作用.

The Serum IL-6 Level is Mediated by the Liver in the Rats with Acute Necrotizing Pancreatitis

Objective To evaluate whether liver plays a protecting role in rats with acute necrotizing pancreatitis(AP),and the role of the liver in mediating serum IL-6 level.Methods The Wistar rats were divided into four groups randomly:control group,potocaval shunting group(PC),AP group,potocaval shunting performed immediately before taurocholate infusion into the pancreatic duct group(PC AP),the survival time was recorded.The same four groups were distributed as in the previous series.In this case,3 hours after the surgical procedure,serum and/or ascites IL-6 levels were measured.Result PC AP group survival times were shorter than the other groups( P< 0.01),but its serum IL-6 level was higher than the other groups( P< 0.05).IL-6 level of ascites in AP group was much higher than serum IL-6 level in every group( P< 0.01),and even higher than IL-6 level of ascites in PC AP group.Conclusion Liver plays a protecting role in rats with acute necrotizing pancreatitis,which may be as a barrier preventing toxins from portal vein into systemic circulation in early stage.
