非糜烂性胃食管反流病的治疗效果和成本分析

目的评价非糜烂性胃食管反流病(NERD)的三种治疗方案对患者生存质量的改善及成本-效果分析。方法NERD患者118例随机分成3组。A组:埃索美拉唑40mg每日1次;B组:埃索美拉唑20mg每日1次;C组:法莫替丁20mg每日2次。3组都同时联用莫沙比利5mg每日3次,疗程均为8周。结果C组4周和8周的症状有效率都显著低于A、B两组(最小χ2=20.311,P=0.000)。A组与B组的症状有效率差别无统计学意义(χ2=0.348,P=0.555)。SF-36量表评分,A组和B组有5个维度积分较治疗前提高,而C组仅有2个维度提高。B组的成本-效果比在3组中最佳。结论对于NERD患者,埃索美拉唑20mg与莫沙比利联用可能为最经济有效地选择之一。

Quality of life and cost-effectiveness of different therapies for non-erosive reflux disease

Objective To evaluate quality of life(QOL) and costeffectiveness of 3 different therapies for non-erosive reflux disease(NERD).Methods 118 NERD patients were randomized to 3 different therapies.Group A: esomeprazole of 40 mg,once daily;Group B: esomeprazole of 20 mg,once daily;Group C: famotidine of 20 mg twice daily.All patients took mosapride of 5 mg,three times daily.The course of treatment was 8 weeks.Results Symptomatic effective rate of 4 weeks and 8 weeks in group C were lower than groups A and B (P<0.05),and there was no statistic difference between group A and group B.Compared with group C,QOL in groups A and B had a striking improvement.5 scales of SF-36 scores had significant improvement in groups A and B after treatment.Group B was most cost effective in 3 groups.Conclusion Esomeprazole Of 2 mg plus mosapride is perhaps one of the most cost effective therapies for NERD patients.
