经颈动脉彩色多普勒超声对缺血性脑卒中早期诊断的价值

目的通过经颈动脉彩色多普勒超声检查121例缺血性脑卒中的患者，分析判断脑卒中早期诊断的指标。方法采取60例无脑卒中者和121例缺血性脑卒中患者，经颈动脉彩色多普勒超声检查颈总动脉（CCA）、颈内动脉（ICA）、颈外动脉（ECA）的内膜增厚、粥样斑块形成、动脉狭窄、颈动脉内膜．中层厚度（IMT）值情况。结果两组无动脉增厚和斑块形成比率有非常显著的差异（P〈0．01）。缺血性脑卒中组管腔狭窄轻度59例、中度32例、严重7例。两组颈动脉IMT值比较，左右CCA、ICA、ECA均有非常显著的差异（P〈0．01）。结论经颈动脉彩色多普勒超声检查IMT值〉1．0mm，动脉粥样斑块2处以上，或硬斑块1处以上，可以预示缺血性脑卒中的发生风险在80％以上。

Early Diagnosis of the Carotid Color Doppler Ultrasound on Ischemic Stroke

Objective To analyze and judge the indicators of a carotid artery color Doppler ultrasound examining 121 cases with ischemic stroke for early diagnosis stroke. Methods 60 cases without stroke and 121 cases with ischemic stroke were examined endometrial thickening, plaque formation, arterial stenosis of carotid artery （CCA）, internal carotid artery （ICA）, external carotid artery （ECA） of, IMT value of the situation by color Doppler ultrasound. Results There were significant difference in no artery thickening and plaque formation in two groups （P 〈 0. 01 ）. Ischemic stroke narrow group had mild lumen 59 eases, moderate 32 cases, serious 7 cases. Carotid IMT value of the two groups was a significant difference about CCA, ICA, ECA （ P 〈 0. 01 ）. Conclusion IMT value 〉 1.0 mm, more than two artery atherosclerosis, or more than a hard plaque were measured by carotid neck color Doppler ultrasound able to indicate the incidence of isehemic stroke risk in more than 80 percent.
