内镜下注射立止血在上消化道大出血中的应用

目的探讨内镜下注射立止血在上消化道溃疡大出血中的止血效果.方法电子胃镜下对出血病灶周边注射立止血.结果48例上消化道溃疡大出血患者,39例一次注射止血成功,5例经第一次注射止血后再发出血,第2次注射止血成功,3例因溃疡出血灶位于十二指肠上角处和胃体中段小弯缘处注射困难或注射效果不佳,给立止血2支加去甲肾上腺素1支作局部喷洒止血成功,1例经注射止血后再发出血而行外科手术治疗.结论胃镜下注射立止血治疗上消化道溃疡大出血,止血效果确切,操作简单,对组织创伤小,无穿孔并发症,尤其适用于中、晚期癌性溃疡出血患者.

Application of Reptilase by Gastroscopy on Upper Alimentary Tract Hemorrhage

Objeotive To explore stanching function of gastroscopy for upper alimentary tract hemorrhage.Method To inject reptilase into periphery of bleeding ulcer by electronic gastroscopy. Results In 48 cases of gastroduodenal hemorrhage, 39 patients were controlled after injecting reptilase once, 5 patients underwent the second injection, 3 patients obtained a goal of stanching by regional spray of combining with noradrenaline for special position and 1 ease accepted surgery for rebleeding. Conclusions To inject reptilase by gastroscopy was useful to manage hemorrhage of cancerous ulcer for advanced grastroduodenal cancer. This method is of good effect, simple operation and microinjury, and no complication of perforation.
