胸部肿瘤患者呼吸道病原菌特征及耐药性分析

目的探讨胸部肿瘤患者呼吸道病原菌特征及耐药性,为术后合理用药提供依据。方法送检胸外科住院患者合格痰液标本793份,进行菌株分离、培养、鉴定和耐药性分析。结果 793份合格痰标本中共检出406株病原菌,检出阳性率51.2%,其中G-杆菌297株,占73.2%,以肺炎克雷伯菌、铜绿假单胞菌、大肠埃希菌为主;G＋球菌78株,占19.2%,以金黄色葡萄球菌为主;真菌31株,占7.6%,以假丝酵母菌为主。药敏试验提示G-杆菌对亚胺培南、美罗培南高度敏感,对氨苄西林、哌拉西林耐药率较高;G＋球菌对万古霉素敏感,对青霉素、红霉素、四环素耐药率较高。结论胸外科肿瘤患者携带多种病原菌,易产生耐药株。术后应根据药敏实验合理选择抗生素。

Analysis on the characteristics and drug resistance of pathogen in respiratory tract in patients with thoracic neoplasms

Objective To explore the characteristics and drug resistance of pathogen in respiratory tract in patients with thoracic neoplasms,and provide the basis of rational use of medicine in postoperation.Methods 793 cases of qualified sputum of the thoracic tumors patients were isolated,cultured,identification and analyzed drug resistance.Results 406 strains of pathogenic bacteria were detected in 793 cases of qualified sputum,the positive rate was 51.2%,of 406 pathogens spectrum,there were 297 strains of G-bacilli（73.2%）,and the main bacteria include Klebsiella pneumoniae,P.aeruginosa and Escherichia-coli.The number of G＋coccis was 78（19.2%）,the main bacterium is Staphylococcus aureus and fungi were 31（7.6%）,the main bacterium is Candida.The results of susceptibility test suggested： the G-bacilli had a highly senstive to Imipenem and Meropenem,and had a highly resistant against Ampicillin and Piperacillin;G＋ coccis had a highly sensitive to Vancomycin,and had a highly resistant against Penicillin,Erythrocin and Tetracycline.Conclusion The patients with thoracic neoplasm carry a lot of pathogens,easy to produce drug-resistant strains.We should according to drug sensitivity test choose the antibiotics.
