失血性休克大鼠血清脂质水平分析

摘    要:目的对失血性休克大鼠血清脂质水平进行检测,探讨脂质代谢在失血性休克发生、发展中的变化规律及机制。方法雄性Wistar大鼠50只,随机分为5组(每组10只):正常对照(SHS)组;重度失血性休克(RHS)组;中度失血性休克(MHS)组,按休克后采血时间的不同又分为休克30 min(MHS-30 min)组、休克60 min(MHS-60 min)组及休克120 min(MHS-120 min)组。分别对各组血清三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C)进行检测。结果 (1)MHS组与SHS组比较:TG显著上升(P〈0.01),TC、HDL-C均显著降低(P均〈0.01),而LDL-C无显著变化(P〉0.05),HDL/LDL比值显著下降(P〈0.01)。(2)RHS组与SHS组比较:TG、LDL-C显著上升(P均〈0.01),TC、HDL-C显著降低(P均〈0.01),HDL/LDL比值显著下降(P〈0.01)。(3)RHS组与MHS组比较:TC、TG均显著降低(P〈0.01);HDL-C及LDL-C无统计学差异(P均〉0.05),而HDL/LDL比值显著下降(P〈0.05)。(4)MHS三不同时间组之间比较:TC水平MSH-30 min组与MHS-60 min组比较无统计学差异(P〉0.05),而MHS-60 min组显著高于MHS-120 min组(P〈0.05);TG水平三组间差异显著,MHS-60 min组最高,分别是MHS-30 min组及MHS-120 min组的2.28倍和1.99倍(P均〈0.01),而MHS-30 min组与MHS-120 min组比较无统计学差异(P〉0.05);HDL-C水平MHS-60 min组显著低于MHS-30 min组(P〈0.05),而MHS-60 min组与MHS-120 min组无统计学差异(P〉0.05);LDL-C水平MHS-60 min组显著高于MHS-30 min组(P〈0.01),而MHS-60 min组与MHS-120 min组无统计学差异(P〉0.05);但HDL/LDL比值三组间均差异显著(P〈0.01或0.05),呈进行性下降。结论不同程度失血性休克大鼠血清脂质水平存在明显差异,而且随着休克的发展脂质水平呈动态变化,血清脂质水平可反映失血性休克的严重程度及其动态进展。

Abstract:Objective To detect the plasma lipid profile of rats subjected to hemorrhagic shock (HS) and analyze the variation and mechanisms of lipid metabolism in occurrence and development of hemorrhagic shock. Methods 50 male wistar rats were randomly divided into 5 groups (n = 10) : sham hemorrhagic shock (SHS) ; refractory hemorrhagic shock (RHS) ; moderate hemorrhagic shock (MHS) which were subdivided into MSH-3Omin, MSH-60min and MSH-120min following the time after shock. The levels of serum total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol (LDL-C) were detected respectively. Results ( 1 ) Compared with SHS, the levels of TG increased, TC, HDL-C and the ra- tio of HDL-C/LDL-C decreased significantly in MHS ( P 〈0. 01 ) ; but no difference of LDL-C was found between them ( P 〉 0.05 ). ( 2 ) Compared with SHS, the levels of TG and LDL-C increased significantly ( P 〈 0.01 ) ; but the levels of TC, HDL-C and the ra- tio of HDL-C/LDL-C decreased (P 〈 0. 01 ). (3) The levels of TC, TG and the ratio of HDL-C/LDL-C decreased significantly in RHS compared with MHS (P 〈 0.01 or 0.05 ) ; but no difference of LDL-C was found between them (P 〉 0.05 ). (4) The levels of TC was higher in MHS-60min than those in MHS-120min ( P 〈 0.05 ) , but no difference of TC was shown between MHS-30min and MHS-60min. The levels of TG in MHS-60min was the highest, they were 2.28 and 1.99 times as mach as those in MHS- 30min and MHS-120min respectively. The levels of HDL-C in MI-IS-60min were lower than those in MHS-30min significantly (P 〈0.05 ) , but no difference was shown between MHS-60min and MHS-120min ( P 〉 0.05 ). The levels of LDL-C in MHS-60min were higher than those in MHS-30min significantly ( P 〈 0.05 ),but no difference was shown between MHS-60min and MHS-120min (P 〉 0.05 ). The difference of the ratio of HDL-C/LDL-C was significant among three MHS groups ( P 〈 0.01 or 0.05 ). Conclusion The serum lipid profile is different in rats subjected to HS of different grade and the lipid levels dynamics vary following the time after shock.
