中央型小细胞肺癌的CT表现及其诊断

目的探讨中央型小细胞肺癌（SCLC）的CT表现及诊断价值。方法对50例资料完整、经病理证实的中央型SCLC的CT征象及临床资料进行了回顾性分析。结果 50例中央型小细胞肺癌CT表现为：（1）肺门区椭圆形或类圆形肿块,边缘清楚、光滑,相应支气管改变轻,肿块包绕支气管形成＂包绕征＂;（2）纵隔淋巴结肿大早而明显;（3）阻塞性改变少而轻。结论肿块包绕支气管征合并广泛的肺门、纵隔淋巴结肿大是中央型SCLC较为特征性的CT表现,结合临床,对本病作出及时诊断是可能的。

Diagnosis and features of CT in central-type small cell lung cancer

Objective To study the features and diagnostic value of CT in central-type small cell lung cancer（SCLC）.Methods CT features and clinical data of 50 cases with central-type SCLC proved by pathology were analyzed retrospectively.Results The CT features of central-type SCLC in 50 cases were：（1）with sharp and clear margin,elliptic and round small hilar mass surrounding unobstructed bronchus,which named ＂encasement sign＂（2）at the earlier stage,hilar and/or mediastinal lymph nodes being obvious;（3） the changes in bronchus obstruction being rare and slight.Conclusion ＂Encasement sign＂ with hilar and mediastinal lymph nodes enlargement may be the specific CT features of central-type SCLC.Based on CT combined with clinical findings,prompt diagnosis of centrall-type SCLC is possible.
