老年人脊髓型颈椎病手术疗效分析

目的 探讨颈椎前路手术治疗老年人脊髓型颈椎病的疗效。方法 回顾性分析了１２１例老年脊髓型颈椎病患者临床资料，术前按日本矫形外科学会（ＪＯＡ）评分平均７．８分。１２１例均行前路减压、植骨融合术，其中４４例行带锁钢板内固定。结果 １２１例中，获得随访１１４例，平均随访时间２９．３个月。有１０１个牢固融合，１３例发生骨不连及假关节形成；采用钢板内固定者１００％获得值骨融合；无内植物并发症。术后ＪＯＡ评分

An analysis of the surgical effect on cervical spondylotic myelopathy in the aged

Objective To analyze the clinical results of anterior approach on cervical spondylotic myelopathy (CSM) in aged patients. Methods The data of 121 cases of CSM were analyzed retrospectively. Mean Japanes orthopedic surgery association (JOA) scale was 7 8 points before operation. All the cases were treated with anterior decompression and bone graft, 44 of them were stabilized with anterior locking plate. Results Two handred and fourteen cases were followed up after operations for an average of 29 3 months. One handred and one cases got solid fusion.Non union and pseudoarthrosis occurred in 13 cases. The fusion rate of the cases stabilized with plating was 100% without hardware failure. After operation the mean JOA scale was up to 10 7 points,with improvement rate of 31 5%. Conclusions Complications of surgery to CSM in aged patients are rather common and the surgical effect is limited. Therefore, surgical indications should be strictly controlled and the peri operative management is important. Anterior approach combined with locking plate fixation is an ideal surgical method to CSM in the aged.
