2018—2020年四川省不同经济区七类癌症患者住院费用及影响因素分析

目的 分析四川省七类癌症患者住院费用情况，为我国医疗控费政策的制定与完善提供数据支撑。方法 在四川省范围内，采用分层抽样抽取12家三级甲等医院，收集2018—2020年肺、乳腺、大肠、胃、食管、肝和宫颈7类癌症患者住院病案首页数据，采用单因素分析和多元线性回归分析，对影响住院费用的因素进行分析。结果 收集癌症住院患者共26 323例，食管癌、结直肠癌和胃癌次均住院费用较高，住院费用均值都在3万元以上，排在前三位。不同癌症类型(β=-0.092,t=-5.611,P<0.001)、住院天数(β=0.709,t=56.511,P<0.001)、是否手术(β=0.701,t=64.495,P<0.001)、医院类型(β=-0.088,t=-4.991,P<0.001)、疾病转归(β=-0.262,t=-17.08,P<0.001)、地区(β=0.073,t=5.989,P<0.001)是癌症患者住院费用的影响因素。结论 通过加强医院综合管理缩短住院天数，可降低癌症患者的住院费用，减轻癌症给患者及社会带来的经济负担。

Hospitalization costs and influencing factors of seven types of cancer patients in different economic regions,Sichuan

Objective To analyze the hospitalization expenses of seven types of cancer patients in Sichuan Province and to provide data support for the formulation and improvement of my country's medical expenses control policy. Methods In Sichuan Province, 12 tertiary first-class hospitals were selected by stratified sampling, and the first page data of hospitalized patients with seven types of cancer, including lung cancer, breast cancer, colorectal cancer, gastric cancer, esophageal cancer, liver cancer and cervical cancer, were collected from 2018 to 2020. The factors affecting hospitalization expenses were analyzed by single factor analysis and multiple linear regression analysis. Results A total of 26 323 cancer inpatients were collected. The average hospitalization expenses of esophageal cancer, colorectal cancer and gastric cancer were higher, and the average hospitalization expenses were more than 30 000 yuan, ranking top three. Different types of cancer(β=-0.092, t=-5.611, P<0.001), hospitalization days(β=0.709, t=56.511, P<0.001), whether surgery(β=0.701, t=64.495, P<0.001), hospital type(β=-0.088, t=-4.991, P<0.001), disease outcome(β=-0.262, t=-17.08, P<0.001), region(β=0.073, t=5.989, P<0.001)were the influencing factors of hospitalization expenses of cancer patients. Conclusion By strengthening the comprehensive management of hospitals and shortening the length of hospitalization, the hospitalization costs of cancer patients can be reduced, and the economic burden of cancer on patients and society can be reduced.
