医疗集团个性化精准帮扶创新研究

目的 探索医疗集团个性化精准帮扶管理模式，强化分级诊疗制度理论研究与实践凝练，巩固拓展健康帮扶成效。方法 深入13家医疗集团成员单位调研，通过问卷调查和个人访谈，广泛收集各单位发展需求。结果 针对共性和个性问题，提出精准帮扶举措，个性化调整优化支持政策，建立了精准帮扶长效机制。结论 项目医院在分级诊疗、健康扶贫等关键环节和重点领域取得实践成效，提高了基层服务能力和百姓就医感受，为“健康中国”战略和“乡村振兴”战略落地提供经验借鉴。

Innovation of Personalized and Precise Assistance of Medical Groups

ObjectiveTo explore the personalized and precise assistance management mode of medical group, strengthen the theoretical research and practice of hierarchical medical system, and consolidate and expand the effect of health assistance. Methods13 member units of the medical group were investigated, and the actual development needs of each unit were extensively collected through questionnaire survey and personal interview.ResultsAccording to their common and individual needs, precise assistance measures were put forward, support policies were personalized adjusted and optimized, and a long-term mechanism of precise assistance was established. Conclusion Project hospital has achieved practical results in hierarchical diagnosis and treatment, health poverty alleviation and other key links and key areas, improved grassroots service capacity and people's feeling of medical treatment, and provided experience for the implementation of "healthy China strategy" and "rural revitalization strategy".
