心肌肌钙蛋白I与慢性心力衰竭的关系

目的探讨血清肌钙蛋白I（cardiac troponin I，cTnI）与慢性心力衰竭（chronic heart failure，CHF）的关系，以及预测慢性心衰预后的临床意义。方法108例慢性心衰患者和38例健康人分别测定血清cTnI浓度、肌酸激酶（creatine kinase，CK）、肌酸激酶同功酶（CK-MB）水平，彩色多普勒测定左心室射血分数（EF）。结果CHF组血清cTnI水平明显高于正常对照组（P〈0．05），cTnI阳性组血清CK、CK—MB均高于cTnl阴性组，但其差异不具有统计学意义（P〉0．05）；cTnI阳性组EF值显著低于cTnI阴性组（P〈0．05）。结论血清cTnI浓度与CHF患者心功能具有显著相关性。CTnI作为评价心力衰竭的一项灵敏度及特异度均较好的指标，cTnI水平可反映心力衰竭的严重程度，能较好反映出心力衰竭患者的心功能，对临床预后具有一定的的预测价值，可作为评估心力衰竭病人预后的指标。

The Relationship between Cardiac Troponin I and Chronic Heart Failure

Objective] To explore the relationship between cardiac troponin I（cTnI） and chronic heart failure（CHF） ,and to study the clinical significance of cTnl on the prognosis of CHF. Methods] Serum level of cTnI, serum activities of ereatine kinase（CK） , creatine kinase-MB（CK-MB） ,and ejection fraction（ EF） monitored for heart function were measured in 108 patients with CHF and 38 healthy cases （ control group ）. Results] Serum level of cTnI was higher in patients with CHF than that in the control group significantly（ P 〈 0. 05 ）. CK and CK-MB in the cTnI increased group were both higher than those in the cTnI normal group （ P 〉 0. 05 ）, and EF in the cTnI increased group was lower than that in the cTnI normal group significantly （ P 〈 0.05 ）. Conclusion] Serum level of cTnI correlated with cardiac function in the patients with chronic heart failure. So the serum cTnl with the better sensitivity and specificity will help to evaluate the severity, cardiac function and prognosis in patients with heart failure ,which can be used as a prognosis predicting indicator for them.
