3种方法治疗输尿管结石528例比较分析

目的探讨气压弹道碎石术与钬激光碎石术治疗输尿管结石的临床疗效。方法回顾性分析我院2010年1月至2012年10月收治输尿管结石528例患者的临床资料。结果钬激光组患者碎石成功率为95.44%,结石排净率为97.61%,气压弹道组患者碎石成功率为88.94%,结石排净率为87.08%,经皮肾镜组患者碎石成功率为98.06%,结石排净率为95.05%,三组之间差异具有统计学意义。三组患者在住院时间、术后并发症方面差异无统计学意义(P>0.05),经皮肾镜组住院花费稍高。结论输尿管镜气压弹道碎石术和钬激光碎石术及经皮肾镜取石术均是治疗输尿管结石的有效方法,输尿管镜钬激光碎石术比气压弹道碎石术具有碎石成功率更高,结石排净率更高的优点,经皮肾镜取石术治疗可作为输尿管上段结石的首选方法。

Comparative Analysis of 528 Cases of Ureteral Stones Treated by Three Kinds Methods

Objeetive To investigate the pneumatic lithotripsy and clinical efficacy of holmium laser lithotripsy for ureteral stones. Methods A retrospective analysis of our hospital patients treated 528 cases of ureteral stonest from January 2010 to October 2012. Results Holmium laser lithotripsy patients 95.44% success rate, stone clearance rate was 97.61%, pneumatic ballistic lithotripsy patients and 88.94% success rate, stone clearance rate was 87.08%, percutaneous nephrolithotomy patients gravel success rate was 98.06%, stone clearance rate was 95.05%, a statistically significant difference between the three groups. Three groups of patients in hospital stay, cost of hospitalization and postoperative complications were no significant differences （P〉0.05）. Conclusion Ureteroscopy pneumatic lithotripsy and holmium laser lithotripsy and percutaneous nephrolithotomy surgery are effective treatment of ureteral stones, Holmium laser lithotripsy than pneumatic lithotripsy has a higher success rate of gravel, higher stone clearance rate advantages, percutaneous nephrolithotomy can be used as the preferred method of treatment of upper ureteral calculi.
