经血管内治疗多发性脑静脉窦血栓

目的：报告３６例多发性脑静脉窦血栓的治疗。方法：随机分为常规治疗和动脉内治疗两组；动脉内治疗采用多次颈动脉内注入尿激酶做溶栓，全身应用华法令抗凝结合抗感染等处理；常规组为静滴蝮蛇抗栓酶和口服新抗凝的方法。结果：动脉组在治疗１０天内全部停用甘露醇且颅内压均降至２．９４ｋＰａ以下；在７天时复查脑血管造影示静脉窦再通。结论：经颈动脉入路注入高浓度纤溶药物，可直接弥散并作用到各静脉窦，而全身有效的抗凝防止了血栓再形成。

Endovascular thrombolytic therapy for extensive thrombosis in cerebral vein sinus

Objective: The therapy of 36 cases with extensive thrombosis of cerebral vein sinus were reported here. Methods:The arterial and conventional groups were randomized. Repeatly thrombolysis with urokinase via internal carotid artery and general anticoagulation with Warfarin as well as anti infection were used in artery group. The Sintrom and Ahalysantinfarctasum were given in conventional group. Results:Mannitol was withdrew and intracranial pressure was decreases lower than 2 94kPa within 10 days after treatment and re angiography showed recanalization of occluded vein sinus in 2 cases after 7 days in artery group. Conclusions:A great doses of urokinase was injected trans internal carotid artery could derectly diffusing each vein sinus for thrombolysis, The effective anticoagulation may prevent from reformed thrombosis.
