急性四肢动脉损伤的诊治

12例急性四肢动脉损伤均据外伤史及临床症状和体征得以确诊，其中7例于伤后8小时内手术，5例于8－24小时内手术。手术方式为：2例行动脉结扎术，2例血管移植术，2例行血管侧壁修补，5例血管端端吻合，1例尺动脉结扎＋桡动脉端端吻合。结果12例患者均痊愈出院。从对12例急性四肢动脉损伤诊治结果分析表明，凡病史明确，结合临床表现确定诊断后，必须及早手术，尽量缩短肢体缺血时间；根据血管损伤情况，选择合适的血管修复方法及积极正确处理合并伤和并发症，是手术治疗获得成功的关键。

DIAGNOSIS AND TREATMENT OF ACUTE ARTERIAL INJURIES OF EXTREMITIES

From 1988 to 1994 twelve patients with acute arterial injuries of the extremities were operated on,Ofwhom, 7 patients were operated within 8 hours after injury and 5 patients between 8 to 24 hours after injury. Theoperations included vascular ligation(n = 2 ), vascular grafting (n = 2 ), vascular repair (n = 2),end-to-endanastomosis (n = 5), ligation of artery plus end-to-end anastomosis (n = 1). All the patients were restored wellafter operation. It indicates that the key to success of operations lies in shortening the time of the extremity is-chemia after injury. selecting the appropriate method to repair the blood vessels, operating debtidement thoroughlyand managing the complications correctly.
