奈达铂联合5-Fu与顺铂联合5-Fu治疗晚期食管癌的疗效对比

目的对比观察奈达铂(nedaplatin,NDP)联合氟尿嘧啶(5-Fu)和顺铂(cisplatin,CDDP)联合5-Fu治疗晚期食管癌的近期疗效。方法采集61例病例,NDP+5-Fu组(NF组)30例,CDDP+5-Fu组(DF组)31例,NF组采用NDP80~100mg/m2,静脉滴注,d1,亚叶酸钙200mg/m2,静脉滴注,d1、2,5-Fu400mg/m2,静脉推注,然后5-Fu600mg/m2持续静脉滴注约22h,d1、2;DF组采用CDDP30mg/m2,静脉滴注,d1~3,其他药物的用法同NF组。结果NF组和DF组的有效率分别为53.33%和35.48%,无显著差异(P>0.05);NF组消化道不良反应小,肝肾毒性轻微。结论NDP治疗晚期食管癌近期疗效不低于CDDP,副作用易于耐受,因此特别适合老年人采用,对CDDP治疗失败或不能耐受CDDP不良反应的病例仍有一定疗效。

Effect Contrasting of Nedaplatin plus 5-Fu and Cisplatin plus 5-Fu in Treatment with Advanced Esophageal Carcinoma

Objective 　To observe the efficacy in the near future of nedaplatin (NDP) and cisplatin (CDDP) for the patient s with advanced esophageal carcinoma. Methods 　Collect 61cases ,30 cases in NDP t reat2 ment group (NF group) ,31 cases in CDDP t reatment group (DF group) ,NF group use NDP 80～100 mg/ m2 ,int ravenous drip ,d1 ,calcium leucovorin 200 mg/ m2 ,int ravenous drip ,d1 ,2 ,then int ravenous bolus of 52Fu 400 mg/ m2 followed by 22 hour infusion of 52Fu 600 mg/ m2 using an ambulatory pump ,d1 ,2 ;DF group use CDDP 30mg/ m2 ,int ravenous drip ,d1～3 ,the usage of other drugs are the same as DF group. Results 　The RR of NF group is 53. 33 % ,the RR of DF group is 35. 48 % ,the difference was not statisti2 cally significant ( P > 0. 05) . There is a lower digestive reaction and lower toxicity of liver and kidney in NF group. Conclusion 　The efficacy in the near future of NF group is higher than DF group and NF group is easier tolerant to the side effect ,so it is fit to the olds especially and do has efficacy to the pa2 tient s failure or cannot tolerate to CDDP.
