高尿酸血症与2型糖尿病血管并发症的相关性

目的研究2型糖尿病（T2DM）患者高尿酸与血管并发症的关系。方法96例2型糖尿病合并高血尿酸患者设为高尿酸组，127例血尿酸正常2型糖尿病为对照组，比较两组血管并发症及各临床生化指标差异，研究尿酸与各指标关系。结果高尿酸组与对照组比较，收缩压、BMI、HbA1c、TG、TC、CRP、IL-6、IL-8及TNF均有统计学差异（P〈0．05），均以高尿酸组较高；糖尿病患者的收缩压、BMI、TG、TC、CRP、IL-6、IL-8及TNF与UA呈正相关（P〈0．05）；高尿酸组血管并发症，冠心病、脑血管病、糖尿病肾病、糖尿病视网膜病变、下肢血管病变发生率均明显高于对照组（P〈0．05）。结论高尿酸血症与2型糖尿病患者的血管病变关系密切，控制2型糖尿病患者机体尿酸水平，对患者血管并发症的防治有重要的临床意义。

Correlation between hyperuricemia and vascular complications in patients with type 2 diabetes mellitus

Objective To explore the relationship between hyperuricemia and vascular complications in type 2 diabetes mellitus （T2DM） patients. Methods Ninety-six T2DM patients combined with high serum uric acid were chosen as high uric acid group,and 127 cases of TD2M patients with normal uric acid were chosen as control group. The vascular complications and clinical biochemical indicators were compared between two groups, and the relationship between uric acid and clinical index was analyzed. Results The systolic blood pressure, BMI, HbAlc, TG, TC, CRP, IL -6, IL -8 and TNF were statistically higher in high uric acid group than in control group（P 〈 0.05）. The systolic blood pressure, BMI,TG,TC, CRP, IL- 6, IL- 8 and TNF were positively related with UA in T2DM patients（ P 〈 0.05 ）. The incidences of coronary heart disease, cerebrovascular disease, diabetic nephropathy, diabetic retinopathy, peripheral vascular disease were statistically higher in high uric acid group than in control group （ P 〈 0.05 ）. Conclusion The hyperuricemia is closely related to vascular lesions in patients with type 2 diabetes. To control the uric acid levels in patients with type 2 diabetes has important clinical significance in prevention and treatment of vascular complications.
