颧骨复合体骨折手术路径探讨

摘    要:目的:总结颧骨复合体骨折复位术的手术路径和效果。方法:对2005-11—2011-05在我院颌面外科救治的资料完整的76例颧骨复合体骨折手术病例的手术路径及治疗效果进行回顾性分析。在76例患者中,有4例颞部切口,有2例前庭沟切口,有26例单纯采用头皮冠状切口,有18例采取头皮冠状切口+下睑缘切口,有9例采取头皮冠状切口+结膜囊切口,有5例采取头皮冠状切口+口腔前庭沟切口,有12例采取头皮冠状切口+口腔前庭切口+下睑缘切口。术后随访平均3年(6~48个月)。结果:76例患者中随访3年的有61例(80.2%),治疗效果Ⅰ级22例(占36.0%);Ⅱ级35例(占57.3%);Ⅲ级4例(占6.3%)。55例(55/61,90.1%)患者取得了良好的治疗效果。结论:冠状切口(必要时附加小切口)和单纯局部小切口在不同适应证中均可获得满意的治疗效果。

Abstract:Objective: To summarize the zygomatic complex fracture operation path and their operative effects.Methods: From 2005-11 to 2011-05,in our hospital and maxillofacial surgery department,76 patients suffered zygomatic complex fractures with surgical reduction were retrospectively analyzed.The selection of incision path were as follows: 4 cases received temporal incision,2 cases with vestibular sulcus incision,26 cases with single coronal incision,18 cases scalp coronal incision + subcilliary incision,9 cases coronal incision + conjunctival incision,5 cases scalp coronal incision + oral vestibule incision,12 cases scalp coronal incision + oral vestibular incision + subciliary incision.Postoperative follow-up period was about 3 years(6~48 months).Results: 61 cases(80.2%) completed followed up period.The treatment effect of grade Ⅰ was observed in 22 cases(36%),grade Ⅱ in 35 cases(57.3%),grade Ⅲ in 4 cases(6.3%).55 cases(55/61,90.1%) patients achieved good therapeutic effects.Conclusion: Coronary incision(addition with small incision if necessary),and small local incision alone may be indicated in surgical reduction and achieve satisfactory results.
