西宁地区睡眠呼吸暂停低通气综合征患者肺功能脉冲强迫振荡检测数据变化的意义

摘    要:目的观察西宁地区(海拔2260m)阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者肺功能检查中强迫振荡(IOS)检测数据的变化,并探讨其临床意义。方法对24例OSAHS患者(OS-AHS组)和12名正常人(对照组)进行常规肺功能及脉冲强迫振荡(IOS)检测。结果OSAHS组中心气道阻抗(CAI)和上气道阻力(R35)明显高于对照组,差别有统计学意义(P〈0.01),中心气道阻力(R20)和外周气道阻力(R5-R20)也高于对照组,差别亦有统计学意义(P〈0.05);将OSAHS组睡眠监测指标与IOS测定指标相关性进行研究,发现呼吸暂停低通气指数(AHI)和R35呈正相关,相关系数(r)分别为0.65和0.66(P〈0.01);夜间最低血氧饱和度(SO2)与CAI和R35呈负相关,r分别为-0.66和-0.65(P〈0.01)。结论IOS技术可作为检测OSAHS患者上气道阻力的一种方法,并且有助于其病理机制的探讨。

Abstract:Objective: To observe the change of measurement values of impulse oscillometry in obstructive sleep apnea and hypopnea syndrome (OSAHS) patients and its clinical significance under high altitude condition. Methods: Routine lung function and impulse oscillometry (IOS) were performed on 24 patients with OSAHS (OSAHS group) and 12 normal subjects (Control group). Results: Central airway impedance (CAI) and upper airway resistance (R35) in OSAHS group were significantly higher than that in control group (P<0.01), meanwhile central airway resistance and peripheral airway resistance (R5-R20) showed higher than the control group (P<0.05). There was a positive correlation between apnea hypoxic index (AHI) and R35 (r=0.65 and r=0.66) (P<0.01), in contrast correlation sleep So2 with CAI and R35(r=-0.66 and-0.65) (P<0.01). Conclusions: IOS technique can be a method for assessing the patients' upper airway abstraction at high altitude, and it is helpful in exploration of the pathological mechanism.
