计算机辅助精子质量分析与常规精液分析的对比性研究

目的评估常规精液分析方法(SRA)和计算机辅助精子质量分析(CASA)各项参数的可信度。方法对145例精液标本在进行CASA的同时,进行SRA分析。SRA按WHO规定的标准和方法进行检测,CASA采用WLJY-9000型伟力彩色精子质量检测系统分析。结果平均精子密度CASA和SRA分别为73.36×106/ml和74.03×106/ml,在分组对60例指标正常精液标本和85例指标异常精液标本的CASA与SRA的精子密度分析中,结果无差异(分别为96.63×106/ml、96.78×106/ml和60.13×106/ml、62.39×106/ml)。对精子活力的分析SRA和CASA则表现出一定差异,145例精液标本CASA精子活力分级为a级25.56%、b级19.58%、c级13.19%、d级41.66%,SRA为a级22.16%、b级20.44%、c级13.71%、d级44.07%,CASA检测a级活力精子比例偏高,d级活力精子偏低,而SRA则相反。结论无论SRA还是CASA对精子密度的分析是可信的,而对精子活力分析时则有差异。因此在判断精子活力时不能仅靠CASA,应同时进行肉眼观察。

Contrast Study on Computer Assisted Sperm Quality Analysis and Routine Sperm Analysis

Objective To evaluate the reliability of semen examination of computer assisted sperm analysis(CASA) and sperm routine analysis(SRA).Method A total of 145 samples of semen were analyzed by CASA and SRA.SRA was tested according to WHO standard and method.CASA used WEILI sperm mass detection system type WLJY-9000.Results The average density of sperm counted by CASA and SRA was 73.36×106/ml and 74.03×106/ml respectively.Comparing the sperm counts of 60 cases in normal group with those of 85 cases in abnormal group by CASA and SRA,no significant difference was observed(the results were 96.63×106/ml,96.78×106/ml and 60.13×106/ml,62.39×106/ml,respectively).There was a certain difference in sperm motility analysis between CASA and SRA.The sperm motility of 145 samples in CASA was 25.56% in Grade a,Grade b 19.58%,Grade c 13.19%,Grade d 41.66%;while in SRA it was Grade a 22.16%,Grade b 20.44%,Grade c 13.71%,Grade d 44.07%.The rate of sperm motility Grade a in CASA was slightly higher,but the rate of Grade d was slightly lower,which was opposite to that in SRA.Conclusions No matter how to analyze sperm count by CASA or SRA,the results are confident,but they are different in sperm motitity analysis.So the judgment of the sperm motility depends on not only CASA,but also macroscopic observation.
