老年糖尿病患者高尿酸血症与胰岛素抵抗的关系探讨

目的探讨老年糖尿病患者高尿酸血症与胰岛素抵抗的关系。方法老年糖尿病患者75例，按照血尿酸（UA）水平分为高尿酸组（HUA）39例和正常尿酸组（NUA）36例，对两组的BMI、腰臀比（WHR）、血UA、TC、TG、LDL-C、糖化血红蛋白（HbA1c）及胰岛素水平进行分析。结果（1）HUA组TG、TC、LDL-C、胰岛素抵抗指数（HOMA-IR）均高于NUA组[（3．39±1．28）mmol／L与（1．39±0．77）mmol／L、（6．61±1．53）mmol／L与（5．15±1．57）mmol／L、（4．28±1．26）mmol／L与（2．364±1．23）mmol／L、（0．85±0．54）与（0．53±0．38）]，差异均有统计学意义（t=2．129、2．012、2．175、2．009，均P〈0．05）。（2）老年糖尿病患者血UA水平与TG、TC、LDL．C及HOMA．IR呈正相关（r=0．327、0．335、0．321、0．331，均P〈0．05）。结论老年糖尿病患者血UA水平与多种因素相关，高尿酸血症与胰岛索抵抗密切相关。

Investigation on the relationship between hyperuricemia and insulin resistance in senile patients with diabetes mellitus

Objective To explore the relationship between hyperuricemia （HUA） and insulin resistance （IR） in senile patients with diabetes mellitus. Methods 75 senile diabetes meUitus patients were divided into HUA group（39 cases） and normal uric acid （NUA） group （36 cases）. The clinical data such as BMI,WI-IR,serum UA, TC,TG,LDL-C, HbAlc and insulin level were analyzed. Results The TG, TC,LDL-C, HOMA-IR in HUA group were significantly higher than those in NUA group [ （ 3.39 ±1.28 ） mmol/L vs （ 1.39 ± 0.77 ） mmol/L, （ 6.61 ± 1.53） mmol/L vs （5.15 ± 1.57） retool/L, （4.28± 1.26） mmol/L vs （2.36 ± 1.23 ） mmol/L, （0.85 ± 0.54） vs （0.53 ± 0.38） ] （ t = 2. 129,2. 012,2. 175,2.009, all P 〈 0.05）. The level of uric acid was positively correlated with TC ,TG, LDL-C and HOMA-IR in senile diabetes meUitus patients （r = 0. 327,0. 335,0. 321,0. 331, all P 〈 0.05 ）. Conclusion The uric acid level of senile diabetes mellitus patients might be related with multiple factors and HUA is closely related with HOMA-IR.
