Th17、Treg细胞在儿童哮喘中的表达及平衡的意义

目的 检测哮喘儿童外周血中Th17细胞（辅助性T细胞17型）、Treg细胞（调节性T细胞）及其相关细胞因子IL-17、IL-10的表达水平,分析并探讨Th17/Treg平衡参与儿童哮喘发病机制中的作用,为儿童哮喘的防治提供新的治疗靶点.方法 收集轻、中、重度哮喘患儿各15例,健康志愿者10例作为对照组,检测外周血Th17细胞及其胞内细胞因子IL-17,Treg细胞及其胞内细胞因子IL-10的表达、比例,进行统计学分析及相关性分析.结果 （1）Th17细胞表达率与IL-17的表达水平,重度、中度哮喘患儿明显高于对照组（均P＜0.05）.（2）Treg细胞表达率与IL-10的表达水平,重度患儿较轻度患儿、对照组高表达（均P＜0.05）,中度、轻度哮喘患儿间差异无统计学意义（均P＞0.05）.（3）Th17细胞、IL-17水平与哮喘患儿的严重程度呈正相关（r=0.753,P＜0.05）,Treg细胞、IL-10水平与哮喘患儿的严重程度呈负相关（r=0.895,P＜0.05）,Th17细胞与Treg细胞在哮喘患儿的严重程度上呈显著负相关（r=0.632,P＜0.01）.结论 在儿童支气管哮喘的发病中,Treg/Th17免疫应答失衡,在一定程度上影响哮喘的严重程度及症状控制.

The clinical significance of the expression and balance of Th17,Treg cells in asthmatic children

Objective To detect the Thl7 cells, Treg cells and related cytokines IL-17, IL-10 in peripheral blood of asthma children,and to explore the role of Thl7/Treg balance in the pathogenesis of asthma,in order to find the new therapeutic target to cure the bronchial asthma. Methods Children with mild,moderate,severe asthma were selected, 15 children in each group, and 10 healthy children were selected as the control group. The Thl7 cells and IL- l7 ,Treg cells and IL-10 in peripheral blood were detected. The data were analyzed. Results （ 1 ） The percentage of Thl7 cells and IL-17 level in severe, moderate asthma children were significantly higher than those in healthy controls （all P 〈 0.05 ）. （2） The percentage of Treg cells and IL-10 level in severe asthma children were significantly higher than those in the mild asthma children and healthy controls（ all P 〈 0.05 ） ,and there were no significant differences between the mild group and the moderate group（ all P 〉 0.05 ）. （ 3 ） The percentage of Thl7 cells and IL-17 level were positively correlated with the severity of asthma （r = 0. 753 ,P 〈 0.05）. The percentage of Treg cells and IL-10 level were negatively correlated with the severity of asthma （r = 0. 895 ,P 〈 0.05 ）. The percentage of Thl7 cells and Treg cells was negatively correlated （r = 0.632,P 〈 0.01 ）. Conclusion The imbalance of Thl7/Treg cells partly affected the severity of acute asthma and the symptom control in the children with acute asthma.
