彩色多普勒超声对肥厚型心肌病患者右心室心肌收缩功能的评价

目的 通过应用CDE观察患者右心室面积变化率（RVPCA）,以评价肥厚型心肌病（HCM）患者的右心室心肌收缩功能（RVSF）.方法 采用CDE检查56例HCM患者（HCM组）及60例健康体检者（对照组）的心尖四腔心观（A4CH）、胸骨旁心底短轴观（PSAX）及左心长轴观（PLAX）三个不同切面RVPCA及其均值.结果 与对照组比较,HCM组PLAX、PSAX、A4CH的RVPCA值均降低,其中PLAX、PSAX的RVPCA值两组比较,差异均有统计学意义（t=17.552、12.473,均P＜0.05）.HCM组综合RVPCA值明显低于对照组（P＜0.01）.HCM组右室游离壁近房室环处心肌纵向收缩期s波波峰速度（Vs）为（10.5±4.1）cm/s,s波速度时间积分（VTIs）为（1.6±0.7）cm;对照组Vs为（13.6±3.8）cm/s,VTIs为（2.3±0.6）cm;两组Vs、VTIs比较,差异均有统计学意义（t=4.262、5.839,均P＜0.01）.结论 HCM患者的右心室心肌收缩功能有一定程度减退,通过CDE对HCM患者RVSF进行评价具有可行性.

Clinical value of color Doppler ultrasound in evaluation of the right ventricular systolic function of patients with hypertrophic cardiomyopathy

Objective To use the color Doppler echocardiography（CDE） to observe right ventricular proportion changed area （RVPCA） of patients with hypertrophic cardiomyopathy（ HCM）, thus to evaluate right ventricular systolic function （RVSF）. Methods Apical four-chamber view （A4CH）, heart parastemal short axis view（PSAX） and left ventrieular long axis view（PLAX） and its mean value of three different facets RVPCA were detected by CDE in 56 HCM patients and 60 healthy persons. Results Compared with the control group, RVPCA values of PLAX, PSAX in HCM group were significantly lower, the differences were statistically significant（ t = 17. 552,12. 473, all P 〈 0.05）. Vs of HCM group was （ 10.5 ± 4.1 ） cm/s, VTIs was （ 1.6 ±0.7） em, Vs of the control group was （ 13.6 ± 3.8 ） cm/s, VTIs was （ 2.3± 0.6 ） cm, the differences between the two groups were statistically significant （ t = 4.262, 5. 839, all P 〈 0.01 ）. Conclusion RVSF of HCM patients diminished to some extent, CDE used to evaluate RVSF of patients with HCM is feasible.
