经胼胝体-穹窿间入路显微手术切除第三脑室肿瘤

摘    要:目的探讨经胼胝体-穹窿间入路显微手术切除第三脑室肿瘤的方法及疗效。方法经胼胝体-穹窿间入路显微手术切除第三脑室肿瘤11例,其中颅咽管瘤5例,星形细胞瘤2例,垂体腺瘤1例,脉络丛乳头状瘤1例,下丘脑错构瘤1例,生殖细胞瘤1例。结果本组9例肿瘤显微镜下全切,2例次全切除,无死亡病例。术后出现多尿、多饮、电解质紊乱5例,中枢性高热2例,嗜睡2例,缄默症2例,经处理后1~2W好转出院。结论经胼胝体-穹窿间入路循解剖间隙进入,直视下操作,对第三脑室周围重要结构损伤较小,肿瘤全切除率较高,术后并发症少,是手术切除第三脑室肿瘤的最佳入路之一。

Abstract:Objective To investigate the methods and effect of microsurgery of third ventricle tuner via transcallosal-interfomiceal approach. Methods Micmsurgery via transcallosal-interforniceal approach was perfomaed in 11 patients with the third ventricle tumors, which included 5 cases of craniopharyngiomas, 2 astrocytomas, 1 pitui- tary adenoma, 1 choroid plexus papilloma, 1 hypothalamic hamartoma, and 1 germinoma. Results Total resection of the tumors was achieved in 9 cases, subtotal resection in 2 cases and no death case. 3he postoperative complications included polyuria, polydipsia, electrolyte disorders in 5 patients, central high fever in 2 patients, lethargy in 2 patients, and mutism in 2 patients, all of whom got improved and discharged after treated for 1 week to 2 weeks. Conclusion TranscaUosal-interfomiceal approach can provide a clear operative field through the anatomical gap under direct vision, and a good protection for the important structures surrounding the third ventricle. Microsurgery via transcallosal-interfomiceal approach has the advantage of higher total removal rate of tuners and fewer complications after operation, so it is one of the best choices for the resection of third ventricle tuners.
