剖宫产术式对前置胎盘患者的影响

目的探讨前置胎盘患者适宜的剖宫产术式。方法分析29例腹膜外剖宫产(A组)及44例腹膜内子宫下段剖宫产(B组)对前置胎盘患者术中及术后的影响，探讨两种手术方式的优缺点。结果切开皮肤至胎儿娩出时间A组较B组显著延长(P＜0.05)；术中出血量在前置胎盘中央型患者A组较B组显著增加(P＜0.05)；胎盘附着子宫前壁及侧壁患者术中出血量A组较B组显著增加(P均＜0.05)；术中产钳助产率A组较B组显著增加(P＜0.05)；术后胃肠蠕动功能恢复时间A组较B组显著缩短(P＜0.05)。结论前置胎盘患者剖宫产以腹膜内子宫下段方式更为适宜。

EFFECTS OF METHODS OF CESAREAN SECTION ON PATIENTS WITH PLACENTA PREVIA

Objective:To search for an appropriate method of cesarean section for patients with placenta previa.Method:73 patients with placenta previa who needed surgical delivery were divided into two groups, 29 with extraperitoneal cesarean section(group A)and 44 with lower uterine segment cesarean section(group B).The intraand postoperation effects on patients were mearsured and analysed to find out the advantages and disadvantages between the two operational methods.Results:The time from skinincision to delivery in group A was remarkably longer than in group B(P<0.05).The amount of hemorrhage during operation in group A(central placenta previa or placenta attached to the anterior and side walls of uterus)was distinctly greater than in group B(P<0.05).The rate of forcepsassisted delivery was higher in group A than in group B(P< 0.05 ),but the restore time of gastrointestinal squirmy function in group A was markedly shorter than that in group B(P< 0.05 ).Conclusion:The results indicated that lower uterine segment cesarean section is the most appropriate surgical method for patients with placenta previa.
