脑梗塞4种药物治疗方案的成本-效果分析

目的 :探讨不同药物治疗方案对同一疾病所产生的经济效果。方法 :对本院2001年3月～2002年2月间脑梗塞住院患者的4种药物治疗方案进行回顾性分析研究 ,筛选出的126例病例根据治疗方案不同分为4组 :复方丹参组 (Ⅰ )、β-七叶皂苷组(Ⅱ )、低分子肝素 (海普宁 ,国产 )组 (Ⅲ )、低分子肝素 (速避凝 ,进口 )组 (Ⅳ )。运用药物经济学成本 -效果分析方法进行比较。结果 :各方案治疗脑梗塞的成本分别为3929 23元、6744 46元、5773 03元、8729 62元 ;有效率分别为70 83 %、84 62 %、87 10 %、88 89 %。每获得1个单位效果 ,4种方案所需花费的成本分别为55 47元、79 51元、66 28元、98 12元。在方案Ⅰ的效果基础上 ,每再获得1个单位效果 ,方案Ⅱ、Ⅲ、Ⅳ所花费的成本依次为204 15元、113 33元、265 80元。结论 :方案Ⅰ为成本 -效果比最低的方案 ,但有效率不高 ;方案Ⅱ、Ⅲ、Ⅳ有效率有较大提高 ,而3种方案中增加相同的效果所需的成本则以方案Ⅲ最低。

Cost- effectiveness Analysis of Four Pharmacotherapeutic Schemes for Cerebral Infarction

OBJECTIVE:To discuss the economic effects of different therapeutic schemes in the treatment of the same illness METHODS:126 cases of cerebral infarction,admitted from March 2001 to February 2002,were collected and divided into four groups:compound salviac miltiorrhizae was given to group Ⅰ,β-escin was given to group Ⅱ,low molecular heparin(domestic) was given to group Ⅲ and low molecular heparin(imported) was given to group Ⅳ Data was evaluated using the pharmacoeconomic cost-effectiveness analysis RESULTS:The costs of four groups were 3 929 23,6 744 46,5 773 03 and 8 729 62 yuans respectively The effective rates of four groups were 70 83%,84 62%,87 10% and 88 89% respectively Four schemes costed 55 47,79 51,66 28 and 98 12 yuans respectively for increasing 1% effective rate CONCLUSION:Among the four schemes,scheme Ⅰ had the lowest cost-effective ratio,but the effective rate was low The effective rates of scheme Ⅱ,Ⅲ and Ⅳ were higher compared with that of scheme Ⅰ In increasing the same efficacy,scheme Ⅲ costed the lowest expense among Ⅱ,Ⅲ and Ⅳ schemes
