123例原发性胆囊癌的临床诊治评析

目的探讨原发性胆囊癌的早期诊断与治疗.方法对该院1997年1月～2005年4月收治的123例、原发性胆囊癌的资料进行回顾性分析应用SPSS10.0统计软件包进行统计学处理.结果至今术前早期诊断仍很困难,影像学检查是主要的诊断方法,其中PET作为新一代综合生物化学的影像学诊断技术大大提高了早期的诊断率,PET诊断符合率为80%,但费用高难以推广.早期原发性胆囊癌手术治疗效果满意,晚期原发性胆囊癌根治术后的平均生存时间较姑息性手术者明显为长.结论早期诊断和治疗是改善原发性胆囊癌预后的关键,原发性胆囊癌的远期疗效仍无显著改善,扩大根治术可望改善其预后,但需慎重选择病人,对晚期原发性胆囊癌应行积极的个体化的综合治疗.

Clinical analysis on 123 patients diagnosis and treatment of primary carcinoma of gallbladder

[Objective] To investigate the diagnosis and treatment of primary carcinoma of the gallbladder (PCG). [Methods] Retrospective analysis of the diagnosis and the clinical data of 123 patients with gallbladder carcinoma treated in our hospital from Jan 1997 to Apr 2005 were retrospective with PCG was carried out. All statistical analyses were performed with SPSS 10. 0 statistical software packane. [Results] Early diagnosis was still very difficult and imaging medicine was the main diagnostic method. The diagnostic rate by PET was 80%. The results of early PCG were satisfactorty, the mean survival time of advanced cases after radical resection was significantly longer. [Conclusions] The long-time survival of the patients with primary carcinoma of the gallbladder is not remarkably improved. With careful patient selection, to improve the prognosis of PCG, early diagnosis and treatment is the key point. Multidisciplinary treatment should be performed for advanced PCG.
