高血压与下肢深静脉血栓形成相关性的病例对照研究

摘    要:目的:通过对患者是否合并高血压、年龄、性别、D-二聚体、同型半胱氨酸、C反应蛋白水平的研究,探讨高血压这一常见基础疾病与下肢深静脉血栓(DVT)形成之间存在何种相关性。方法:选取山西医科大学第一附属医院普外科在2008年6月至2013年5月入住并确诊为DVT的病人71例作为病例,随机选择同时期入住该院的非深静脉血栓形成病人71例作为对照。结果:病例组D-二聚体、C反应蛋白、同型半胱氨酸水平显著高于对照组(P〈0.05)。有高血压史的人群患DVT的危险性是非高血压人群患DVT的4.399倍,D-二聚体不正常的人群患DVT的危险性是正常人群的3.483倍,同型半胱氨酸不正常的人群患DVT的危险性是正常人群的4.337倍,C反应蛋白不正常的人群患DVT的危险性是正常人群的5.632倍。结论:下肢深静脉血栓形成好发于中老年人,高血压史增加了下肢深静脉血栓形成的危险性,D-二聚体、同型半胱氨酸、C反应蛋白可能与下肢深静脉血栓形成有关,高血压中D-二聚体、同型半胱氨酸、C反应蛋白水平的升高增加了患DVT的风险。

Abstract:Objective: To explore the correlation between hypertension,a common underlying disease,and deep vein thrombosis( DVT) in lower limbs through the assessment of hypertension,age,gender,and the levels of D-dimer,homocysteine and the C-reactive protein. Methods: 71 inpatients,hospitalized and diagnosed as DVT from June 2008 to May 2012 in Department of general surgery,the first affiliated hospital of Shanxi medical university,were taken as subject cases. 71 inpatients without DVT, hospitalized in the same period in this hospital,were selected at random as the controls. Results: The levels of D-dimer,the C-reactive protein and homocysteine in the patient group were higher than those in the control group( P 0. 05). The risk of DVT in the people with a history of high blood pressure was 4. 399 times that of DVT in those without hypertension. The risk of DVT in those with abnormal D-dimer was 3. 483 times that of DVT in those with normal D-dimer. The risk of DVT in those with abnormal homocysteine was 4. 337 times that of DVT in those with normal homocysteine. The risk of DVT in those with abnormal C-reactive protein was 5. 632 times that of DVT in those with normal C-reactive protein. Conclusion: DVT in lower limbs occurs more commonly in the aged. The history of high blood pressure increases the risk of deep vein thrombosis in lower limbs. D-dimer,homocysteine and C-reactive protein may be correlated to deep vein thrombosis in lower extremity. The rise in the levels of D-dimer,homocysteine and C-reactive protein in those with hypertension can increase the risk of DVT.
