阿罗洛尔、咪唑普利和依那普利治疗原发性高血压疗效观察(附24小时动态血压结果分析)

目的 评价阿罗洛尔、咪唑普利、依那普利对24 h血压的降压效果。方法 轻、中度原发性高血压患者60例,采用随机区组法分为三组,每组20例,使用24 h动态血压监测方法对阿罗洛尔(20-30mg/d)、咪唑普利(5-10mg/d)及依那普利(5-1Omg/d)降压疗效进行为期六周的对比现察。结果 三种药物对诊室血压均有明显降压效果(P<0.001),组间无明显差别(P>0.05)。动态血压结果显示三种药物均有明显降低动态血压的效果(P<0.05),阿罗洛尔组降低24 h平均血压及白天平均血压幅度比其它两组大(P<0.05)。三种药物的谷/峰比值均大于50%。治疗前后的血压曲线,阿罗洛尔组呈完全分离状态,咪唑普利组、依那普利组均有交叉重叠现象,依那普利组重叠点较多。结论 阿罗洛尔对控制白天血压较为理想,长效制剂咪唑普利能平稳地控制24 h血压。

The efficacy of arotinolol and imidapril versus enalapril in treatment of hypertension by 24-hour ambulatory blood pressure monitoring.

Objective To evaluate the efficacy of decreasing 24-hour blood pressure with arotinolol. imidapril and enalapril. Methods The study was carried out in 60 patients with mild to moderate essential hypertension. They were randomized blockedly into three groups (arotinolol group 20-30mg/d, imidapril group 5-10mg/d , enalapril group 5-10mg/d), which were assesed by 24-hour ambulatory blood pressure (ABP). Results After six weeks monotherapy, the three drugs induced significant and similar extent reduction in clinical blood pressure (CBP)(P>0. 05). They induced siginificant reduction degree of ABP(P<0. 05),meanwhile that of 24-ABP and day-ABP in the arotinolol group was greater than that in the other groups(P<0. 05). The trough:peak ratio value of each group was more than 50%. The blood pressure curve in arotinolol group was separated completely, the curves both in imidapril and enalapril groups had much intersection and overlapping, which was much more in the later. Conclusion Arotinolol is better to control day-ABP. As a long acting drug,imidapril can do better to control 24-ABP steadily.
