体外受精-胚胎移植成熟卵的影响因素分析

目的探讨体外受精-胚胎移植（IVF-ET）/卵泡浆内单精子显微注射（ICSI）周期中成熟卵的影响因素及与妊娠结局的关系。方法对IVF/ICSI的112个周期进行回顾性分析,按照获成熟卵的数量分为3组,A组：1-4个,B组：5-10个,C组：≥11个,分析其与妊娠结局的关系。结果 IVF/ICSI周期中所获MII卵数量变异的65.4%是由年龄、BMI、不孕年限、HCG日和采卵日的E2、P的水平起主要决定作用。BMI、HCG日E2及采卵日P水平越高的患者获得的MII卵数量越多且差异有统计学意义（P〈0.05）。尽管MII卵在A组的年龄显著高于C组,C组的妊娠率最高,A组最低,但三组间的妊娠率差异无统计学意义（P〉0.05）。结论 IVF/ICSI周期所获成熟卵数量与BMI、HCG日E2及采卵日P水平密切相关,但其不影响新鲜周期的妊娠结局。

The analysis of effects on metaphase II oocyte in vitro fertilization-embryo transfer

Objective To investigate the related factors which affect the metaphase Ⅱoocyte and the relationship between the factors and the pregnancy outcome in IVF/ICSI cycle.Methods 112 IVF/ICSI cycles were divided into three groups according to the number of the MⅡ oocyte,group A： 1≤MⅡ≤4,group B： 5≤MⅡ≤10,group C： MⅡ≥11.To analyze the relationship between MⅡ oocyte and pregnancy result.Results 65.4% of the number of MⅡ oocyte were depended on the age,BMI,duration of fertility,and the level of the E2 and P on HCG and ovum pick-up day.However,the number of MII oocyte was significantly higher when BMI,the level of the E2 on the day of HCG and P on ovum-pick up day were rising（P0.001）.Although the age of fertility women was significantly higher in group A than that in group C,and there was no significant difference between three groups in pregnancy rates（P0.05）.Conclusion The number of MII oocyte is closely related with BMI,the level of the E2 on the day of HCG and P on ovum pick-up day,but it does not affect the pregnancy outcome in a fresh cycle.
