光棒引导与直接喉镜气管插管临床效果比较

摘    要:目的比较光棒引导和直接喉镜气管内插管成功率、口腔损伤和血流动力学的变化,评价光棒在临床麻醉中的应用效果。方法 80例ASA评分Ⅰ~Ⅱ级的外科手术患者随机分为光棒组(A组,n=40)和直接喉镜组(B组,n=40)。根据Mallampati分级气道分为Ⅰ~Ⅳ级,记录插管成功率、插管时间,记录诱导前、诱导后、气管插管即刻、气管插管后1、3min的SBP、DBP、HR的变化和术后咽喉痛、声音嘶哑等并发症。结果两组插管成功率均为100%。A组插管时间短于B组插管时间(P值=0.001);光棒插管对循环功能的干扰小,术后并发症少。结论光棒引导插管成功率高、口腔损伤少、对血流动力学影响较轻,是一种成功率高、损伤轻、安全、有效、轻便的气管插管器具。

Abstract:Objective To compare the success rate of intubation,the injuries to oral cavity and the hemodynamic changes during oral tracheal intubation with the Lightwand and the Macintosh direct laryngoscope.Methods Eighty patients ASA physical status Ⅰ ~ Ⅱ,aged 31 ~ 75 years,scheduled for elective plastic surgery under general anesthesia requiring orotracheal intubation were randomly allocated to either the Lightwand group(group A) or the Macintosh direct laryngoscope group(group B).Results Successful intubation occurred in 80 of the patients both in the group A and group B.The time used for a complete intubation of the group A was shorter than the group B。The changes of hemodynamic parameters(SAP,MAP,HR) were significant in group B than in group A(P < 0.05).The interference of lightwand against hemodynamic circulatory was slight and complications afterwards were low.Conclusion Compared with the macintosh direct laryngoscope,tracheal intubation using Lightwand is more successful,safer,valid and less injurious.
