40岁以下年轻妇女子宫内膜癌10例临床分析

目的:探讨年轻妇女子宫内膜癌的临床特征,诊断、病理特点,治疗及预后的特点。方法:对1995年1月~2004年6月该院收治的10例40岁以下年轻妇女(包括40岁)子宫内膜癌患者进行回顾性总结分析。结果:1995年1月~2004年6月该院共收治94例子宫内膜癌患者,其中40岁以下年轻妇女10例,占10·64%。除1例晚期保守治疗病人死亡外,余9例手术治疗随访至今仍健在。结论:40岁以下年轻妇女子宫内膜癌多与月经不调、不孕不育、多囊卵巢综合征等高危因素有关;多不具备子宫内膜癌三联征的典型表现;且多数为高分化腺癌,与子宫内膜腺瘤增生及重度非典型增生难以区别;以手术治疗为主,预后比老年者好。对于有生育要求的可酌情保守治疗。

Clinical analysis of 10 cases on endometrial carcinoma in women aged 40 years or younger

Objective:To investigate the clinical char ac teristic, diagnosis, pathologic characteristics, therapy and prognosis of endome trial carcinoma (EC) of in women aged 40 years or younger. Methods:10 cases of EC of women aged 40 years or younger a dmitted in this hospital from January 1995 to June 2004 were retrospectively ana lyzed. Results:Of 10 cases accounting for 10.64% (10/94), 9 case s treated by surgery survived, except one case treated by conservative therapy d ied. Conclusion:The high risk factors of EC in women aged 40 ye ars and younger include menstrual dysfunction, sterility, polycystic ovarian syn drome (PCOS). Most of EC are highly differentiated adenocarcinoma without typic al clinical symptom of triad of EC, which has difficult in differenting from end ometrial adenomatous hyperplasia and high-grade atypical hyperplasia. The main therapy is surgery, the prognosis of the young women with EC is better than that of the old. For those with requirement of pregnancy, the conservative therapy should be considered.
