心肌肌钙蛋白Ⅰ及心肌酶谱在诊断支原体肺炎合并心肌损伤中的临床价值

目的探讨心肌肌钙蛋白Ⅰ(cTnI)、肌酸激酶同工酶(CK-MB)在支原体肺炎合并心肌损伤中的临床诊断价值。方法检测68例支原体肺炎(Mycoplasma-pneumoniae MP)患儿和20例正常对照儿童心肌肌钙蛋白(cTnI)、肌酸激酶同工酶(CK-MB)、肌酸激酶(CK)、乳酸脱氢酶(LDH)浓度,并将68例支原体肺炎患儿分为心脏病变组(A组)33例和无心脏病组(B组)35例,比较cTnI、CK-MB的敏感性和特异性。结果 MP组与对照组血清(CK)、乳酸脱氢酶(LDH)浓度比较,差异无统计学意义(P>0.05),而血清cTnI、CK-MB浓度明显高于对照组(P<0.01)。在诊断支原体肺炎合并心肌损伤中cTnI的敏感度(84.85%)和特异度(82.86%)均高于CK-MB的敏感度(66.67%)和特异度(77.14%)。结论 cTnI、CK-MB为诊断支原体肺炎合并心肌损伤的特异性指标,但cTnI具有更高特异性和敏感性。

Clinical assessment of myocardial cardiac troponin I(cTnI)and enzyme spectrum for the detection of mycoplasma-pneumoniae mergence with cardial injury

Objective To explore the clinical assessment of serum cardiac troponin Ⅰ and CK-MB for the detection of mycoplasma-pneumoniae（MP）mergence with cardial injury.Methods Serum levels of cTnI,CK-MB,creatine kinase（CK）,lactic dehydrogenase（LDH）were determined in 68 children with MP and 20 controlled children without disease respectively.68 children with MP were divided into A group with myocardial injury and B group without myocardial injury.The sensitivity and specificity of cTnI,CK-MB were compared.Results There were no significant difference in serum levels of CK,LDH between MP group and the controlled group（P〈0.05）.The levels of serum cTnI and CK-MB in MP group were significant higher than the controlled group（P〈0.01）.The sensitivity was cTnI（84.85%）CK-MB（66.67%）,and the specificity was cTnI（82.86%）CK-MB（77.14%）.Conclusion The serum levels of cTnI,CK-MB are specific for diagnosis of myocardial injury in mycoplasma-pneumoniae mergence with cardial injury,and the determination of cTnI is more sencitivity and specificity.
