亲水性聚丙烯酰胺凝胶腔内注射治疗女性压力性尿失禁

目的 评估亲水性聚丙烯酰胺凝胶(生物凝胶)经尿道腔内注射治疗女性压力性尿失禁的疗效。方法 24例女性压力性尿失禁患膀胱镜直视下于后尿道近膀胱开口0．5-lcm的3,6，9点处黏膜下穿刺注射生物凝胶平均12．2(6．20)ml／次。结果 腔内注射一次18例，二次5例，三次1例，累积平均注射量18．9(6—38)ml。术后尿道平均延长1．03cm，膀胱颈部尿道121周径平均缩小3．1lcm，症状消失13例，占54．2％；改善11例，占48．5％；无并发症和不良反应发生。结论 生物凝胶腔内注射治疗女性压力性尿失禁疗效确切，创伤微小，可重复注射，可选择为压力性尿失禁的外科治疗方法。

Periurethral hydrophilie polyacrylamide injection in the treatment of female stress urinary incontinence

AIM To evaluate the treatment outcome of transurethral submucosal hydrophilic polyacrylamide injection to the female stress urinary incontinence. METHODS 24 women with stress urinary incontinence were treated with transurethral submucosal hydrophilic polyacrylamide injection by cystoscope. Hydrophilic polyacrylamide was injected at the 3,6 or 9 O'clock position in an area about 0.5-1cm distal to the bladder neck and the average volume of hydrophilic polyacrylamide injected was 12.2 (range 6-20)ml each time. RESULTS Of 24 patients, 18 received one injection, 5 two injections and only 1 case received three injections. The average total volume of hydrophilic polyacrylamide injected was 18.9 (range 6-38) ml. The urethra was lengthen average 1.03cm and the circumference of the bladder neck was narrowed average 3.11cm after the injections. 13 cases (54.2%) were cured and 11 cases (48.5%) were improved and no complication and side effects were noted in follow-up. CONCLUSION Transurethral hydrophilic polyacrylamide injection is safety and efficacy to treat female stress urinary incontinence. It can inject repeatedly if necessary. Submucosal hydrophilic polyacrylamide injection is an acceptable alternative surgery in the treatment of stress urinary incontinence.
