摘    要:目的:探讨神经源性膀胱排尿异常的尿动力学表现。方法:49例不同原因导致神经源性膀胱患者,其中尿潴留组24例,尿失禁组25例。对其自由尿流率、膀胱压力容积/压力流率/肌电图联合测定和静止期尿道压力进行测定。结果:尿潴留组尿动力学主要表现为尿流率降低,残余尿量增加,大容量膀胱,逼尿肌无反射,高逼尿肌漏尿点压;而尿失禁组尿动力学主要表现为逼尿肌反射亢进和逼尿肌括约肌协同失调。结论:对神经源性膀胱排尿异常患者进行尿动力学检查有助于对其进行尿动力学分类,指导临床治疗。

Abstract:Aim: To study urodynamic change of the patients with neurogenic bladder and voiding dysfunction.Methods:The results of free uroflowmetry, filling cystometry/pressure flow/electromyography and static urethral pressure measure were analyzed in 49 patients with neurogenic bladder and voiding dysfunction due to different diseases. Results:In the retention group the patients represented decreased flow rate, increased residual urine volume and maximum cystometric capacity, detrusor areflexia,and high detrusor leak point pressure; but in the incontinence group they represented detrusor hyperreflexia and detrusor sphincter dyssynergia.Conclusion:It is suggested that the urodynamic study can help clinical doctors to classify and treat patients with neurogenic bladder and voiding dysfunction.
