后路椎弓根螺钉内固定治疗胸腰椎爆裂骨折

目的探讨应用后路椎弓根内固定治疗胸腰椎爆裂骨折的效果。方法采用后路椎弓根内固定治疗胸腰椎爆裂骨折患者53例。结果全部病例随访8～54个月,伤椎椎体前、后缘高度术前平均37.2%（12%～51%）和平均78.3%（32%～87%）,术后恢复至平均94.3%（92%～97%）和平均97.5%（92%～99%）。术前Cobb角平均23°（13°～38°）,术后平均5.1°（0°～10°）。脊髓神经损伤者术后按美国脊髓损伤协会（A-SIA）标准均恢复1级以上。结论手术治疗胸腰椎爆裂骨折可有效恢复脊柱序列及椎管容积,矫正后凸畸形,有利于脊髓神经功能恢复。

Treatment of thoracolumbar burst factures with posterior pedicle screw fixation

Objective To study the clinical effect of thoracolumbar burst fractures treated by posterior pedicle screw fixation. Methods 53 cases with thoracolumbar burst fractures were treated with posterior pedicle screw fixation. Resuits All the patients were followed up for 8 - 54 months. The mean heights of anterior and posterior edge of vertebral body restored respectively from 37.2% （ 12% - 51% ）and 78. 3% （32% -87% ）preoperatively to 94. 3 % （92% -97% ） and 97. 5% （92% - 99% ）postoperatively, and mean Cobb angles from 23 （13 - 38 ）degrees to 5. 1 （0-10） degrees. The patients with spinal cord and nerve injured were all recovered to over grade I by ASIA. Conclusions Sugical treatment is effective to thoracolumbar burst fractures, which can restore the spinal alignment and volume of spinal canal and correct the kyphosis for the recovery of thoracolumbar function.
