小剂量肝素钠对过敏性紫癜患儿血浆IL-6、IL-8的动态影响

目的 观察过敏性紫癜(HSP)患儿血浆IL-6、IL-8的改变以及应用小剂量肝素钠后的动态变化.方法 HSP患儿56例,其中肝素治疗组(肝素组)30例,基础治疗组(基础组)26例,两组治疗前及治疗后3、7 d、停药后3 d采用酶联免疫法测定血浆IL-6、IL-8水平.结果 HSP组血浆IL-6、IL-8明显高于健康对照组,均有统计学意义(P均＜0.01);HSP基础组治疗前、后血浆IL-6、IL-8无统计学意义(P均＞0.05);HSP肝素组治疗3 d后血浆IL-6、IL-8开始下降,但与基础组比较差异无统计学意义(P＞0.05),治疗7 d及停药后3 d下降明显,与基础组比较差异有统计学意义(P均＜0.01).结论 细胞因子异常可能参与HSP及紫癜性肾炎的发病过程,早期予小剂量肝素钠治疗可以影响患儿血清中IL-6、IL-8的水平,肝素可能能减轻过敏性紫癜患儿的肾损伤.

Changes of interleukin-6,interleukin-8 and effect of interfering therapy with minidose heparin in children with Henoch-Schonlein purpura

Objective To observe the levels of plasma interleukin-6(IL-6) and interleukin-8(IL-8) in children with henoch-schonlein purpura(HSP) and the changes of parameters above after therapy with minidose heparin.Methods Fifty-six HSP patients,heparin group(n=30) and conventional therapy group(n=26),were studied.The levels of plasma IL-6 and IL-8 were observed with ELISA assay in 10 days before and 3 days,7 days after therapy,3 days after stopping medication.Results The levels of plasma IL-6 and IL-8 in HSP patients were obviously higher than those in control group(all P<0.01).IL-6 and IL-8 in HSP patients decreased remarkably 7 days after heparin therapy and 3 days after stopping medication(all P<0.01).Conclusion Abnormal cytokine may involve in the development of HSP and HSP nephritis.The levels of plasma IL-6 and IL-8 can be decreased after therapy with minidose heparin.Heparin may prevent HSP nephritis injury.
