高频彩色多普勒超声在甲状腺癌诊断中的应用

摘    要:【摘要】目的探讨高频彩色多普勒超声在甲状腺癌中的诊断价值。方法选择2010年1月~2011年12月在本院应用彩色多普勒超声检查并经手术、病理及实验室检查证实的甲状腺癌患者20例进行观察。结果甲状腺癌二维超声图像上表现多为不均质低回声,形状不规则,可为锯齿状或蟹足状,甚至与周围组织分界不清;7例周边伴蟹足样回声,6例有不规则液性暗区,无一例表现为高回声,8例伴颈淋巴结肿大。彩色多普勒血流显示血流信号丰富,其中,Ⅲ级12例(60.0%),血流呈高阻力型(RI〉0.7)1l例(55.0%)。肿瘤侧的Vmax、Vmin、RI均明显高于无瘤侧,差异有统计学意义(P〈0.05)。结论彩色多普勒超声在甲状腺癌诊断中具有较好的应用价值,值得推广和应用。

Abstract:Objective To investigate the diagnostic value of high-frequency color Doppler ultrasound for thyroid can- cer. Methods Twenty cases of thyroid tumors confirmed by surgery, pathology and laboratory tests were observed by color Doppler ultrasound in our hospital from January 2010 to December 2011. Results The two-dimensional ultra- sound image of thyroid cancer had not homogeneous hypoeehoic and irregular shape, such as jagged or Crab-like, even with the unclear surrounding tissue boundaries; 7 cases surrounded with crab-like echo, 6 cases had irregular liquid dark area, no case showed hyperechoic, 8 cases combined with cervical lymphadenopathy. Color Doppler flow displayed rich blood flow signals, grade IU had 12 cases (60.0%), high resistance type blood flow (RI〉0.7) occurred in 11 cases (55.0%). The Vmax, Vmin, RI of tumor side were significantly higher than those of the non-tumor side, the difference was statistically significant (P 〈 0.05). Conclusion Color Doppler ultrasound in the diagnosis of thyroid cancer has good application value, and it is worthy of promotion and application.
