围绝经期妇女情绪障碍相关影响因素分析

目的:探讨围绝经期妇女情绪障碍的相关因素,为防治情绪障碍提供依据。方法:通过抑郁自评量表(SDS)和焦虑自评量表(SAS)来筛选病例,采用病例对照的研究方法,对病例组和对照组分别采用汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)、生活事件量表(LES)、艾森克人格问卷(EPQ)、应付方式问卷测评,同时测量研究对象的激素水平,从多因素角度对围绝经期情绪障碍问题进行研究。结果:多因素分析结果显示,影响围绝经期情绪障碍的因素包括HAMD总分(OR=1.677)、HAMA总分(OR=1.581)、负性生活事件(OR=2.088)、神经质(OR=1.602)、自责(OR=1.859)为危险因素,而雌二醇(E2)(OR=0.632)、5-羟色胺(5-HT)(OR=0.598)为保护因素。结论:影响围绝经期妇女情绪障碍的危险因素较多,涵盖生理、心理、社会等诸多因素,除了传统的激素替代疗法外,还应重视通过心理、精神药物治疗。

Analysis on related influencing factors of emotional disorder among perimenopausal women

Objective:To explore the related factors of emotional disorder among perimenopausal women,and provide a basis for preventing and treating emotional disorder.Methods:SDS and SAS were used to screen cases,a case-control study was performed,HAMD,HAMA,LES,EPQ,and coping style questionnaire were used to evaluate the cases in case group and control group;the levels of hormones in study objects were detected;emotional disorder during perimenopausal period was researched from multi-factor perspective.Results:The results of multivariate regression analysis showed that the influencing factors of emotional disorder during perimenopausal period included total score of HAMD(OR=1.677),total score of HAMA(OR=1.581),negative events(OR=2.088),nervousness(OR=1.602),self-accusation(OR=1.859),which were risk factors;while estradiol level(OR=0.632) and 5-hydroxytryptamine(5-HT)(OR=0.598) were protective factors.Conclusion:There are many factors affecting female emotional disorder during perimenopausal period,covering physiological,psychological,social factors,and so on.Except for traditional hormone replacement therapy,more attention should be paid to mental treatment and psychotropic drug therapy.
