术前新辅助化疗治疗肺癌的副作用及手术麻醉管理进展

肺癌目前是恶性肿瘤首位死亡原因,手术是非小细胞肺癌治疗选择之一,但仅20%的患者可行手术治疗。随着医学技术的迅速发展,人们对非小细胞肺癌的治疗又有了新认识。引入了术前诱导化疗（又称新辅助化疗）概念,是使患者获得手术切除肿瘤机会的一种手段。术前化疗虽然有很大优势,但其毒副作用不容忽视,化疗药物除了对肝、肾功能有影响外还对肺、心、神经及血液有毒性。因此,肺癌治疗水平的更新要求麻醉技术水平进一步提高。本综述对化疗后肺癌手术患者的麻醉管理,包括麻醉前评估与指导、麻醉前气管导管的选择及术前术后各项管理等进展做一介绍。

Side effect of preoperative neoadjuvant chemotherapy on lung cancer patients and the progress of anesthesia management

Now, lung cancer becomes the first death cause of malignant tumors. Non-small cell lung cancer operation is one treatment option, but only 20% of the patients have the opportunity of operation treatment. With the development of medical technology is developing rapidly, people on the treatment for non-small cell lung cancer have new study. Introducing the preoperative induced chemotherapy （neoadjuvant chemotherapy） is a concept, so that patients receive a chance of operation resection of the tumor. Preoperative chemotherapy has a lot of advantages, but its adverse reaction can not be ignored, chemotherapy drugs have more toxicity to liver, kidney function influence also to lung, heart, nerve and blood. Therefore, lung cancer treatment update require anesthesia techniques to further more. This review on the anesthetic management which include anesthesia evaluation and guidance, tracheal catheter selection and all management at pre- or post-operative of patients after chemotherapy with lung cancer and other aspects of progress to make introduction.
