SPR-HPV技术检测宫颈人乳头瘤病毒感染状况的临床分析研究

目的:用SPR-HPV技术了解重庆医科大学附属第一医院就诊女性人乳头瘤病毒(HPV)感染状况。方法:采用SPR-HPV技术检测1 000例女性宫颈脱落细胞24种HPV基因亚型。结果:HPV总感染率为28.2%(282/1 000),22种亚型被检出,单一感染211例(21.1%),其中细胞学检查阳性94例;复合感染71例(7.1%),其中细胞学检查阳性32例,单一感染与复合感染差异无统计学意义(χ2=0.005 5,P>0.05);单一感染211例中高危型HPV感染186例,低危型HPV感染25例;单一感染中低危阳性者细胞学检查阳性4例,单一感染中高危阳性者细胞学检查阳性90例,两者比较差异有统计学意义(χ2=9.37,P<0.005)。单一感染211例中感染HR-HPV基因型前4位者依次为HPV16、58、52、51,其中HPV16感染54例(25.29%)。结论:重庆医科大学附属第一医院就诊女性HPV感染率较文献报道的全国其他地区位于中等偏下水平,以单一、高危型感染为主。

Clinical analysis and research on infection status of human papillomavirus detected by SPR-HPV technology

Objective:To understand the infection status of human papillomavirus(HPV) in female patients from the hospital by SPR-HPV technology.Methods:SPR-HPV technology was used to detect 24 kinds of HPV gene subtypes in cervical exfoliative cells and genital tract secretions of 1 000 women.Results:The total HPV infection rate was 28.2%(282/1 000),22 subtypes were detected;211 women(21.1%) were found with single infection of HPV,including 94 women with abnormal cytological results;71 women(7.1%) were found with multiple infection of HPV,including 32 women with abnormal cytological results;there was no statistically significant difference between the women with single infection of HPV and the women with multiple infection of HPV(χ2=0.005 5,P>0.05);among 211 women with single infection of HPV,186 women were found with high risk HPV infection,including 90 women with abnormal cytological results;25 women were found with low risk HPV infection,including 4 women with abnormal cytological results;there was statistically significant difference(χ2=9.37,P<0.005).Among 211 women with single infection of HPV,the top four high risk HPV genotypes were HPV16,HPV58,HPV52,and HPV51,54 women(25.29%) were found with HPV16 infection;HPV single infection was the main type among HPV infection,among 151 women with abnormal cytological results,94 women were found with single infection of HPV.Conclusion:The infection rate of HPV among women from the hospital was at a middle level compared with other areas in China,single infection and high risk infection were the main type.Appropriate HPV screening should be conducted among the cases receiving opportunistic screening in clinic,and regular follow-up should be performed,which is helpful to prevent and find cervical malignant lesions timely and reduce the damage of cervical malignant lesions to women.
