糖尿病合并妊娠43例临床分析

目的:探讨糖尿病合并妊娠的孕产期管理。方法:对顺德区第一人民医院2003年1月～2010年12月的糖尿病合并妊娠孕妇的妊娠及分娩结局进行回顾性分析,并和8年来分娩的妊娠期糖尿病的母婴结局进行比较。结果:糖尿病合并妊娠的发生率为0.13%。糖尿病合并妊娠剖宫产率、巨大儿、产伤、流产、死胎、畸胎、酮症酸中毒发生率高于妊娠期糖尿病,其他母婴结局与妊娠期糖尿病无明显差异。结论:对糖尿病合并妊娠加强孕前监测、孕期应用胰岛素控制血糖、密切胎儿监测,适时终止妊娠可获得良好的母婴结局。

Clinical analysis on 43 cases with diabetes mellitus combined with pregnancy

Objective:To explore maternal management of diabetes mellitus combined with pregnancy.Methods:The pregnant and delivery outcomes of pregnant women with diabetes mellitus from the hospital from January 2003 to December 2010 were analyzed retrospectively,then the results were compared with maternal-infantile outcomes of patients with gestational diabetes mellitus.Results:The incidence rate of diabetes mellitus combined with pregnancy was 0.13%.The rate of cesarean section,incidence rates of macrosomia,birth injury,abortion,stillbirth,fetal malformation,ketoacidosis in patients with diabetes mellitus combined with pregnancy were higher than those in patients with gestational diabetes mellitus,but there was no significant difference in the other maternal-infantile outcomes between them.Conclusion:For the patients with diabetes mellitus combined with pregnancy,enhancing pregestational monitoring,controlling blood glucose by insulin during pregnant period,monitoring fetuses closely,and terminating pregnant women appropriately can achieve good maternal-infantile outcomes.
