鞍结节脑膜瘤的显微外科治疗（附65例报告）

目的 提高鞍结节脑膜瘤的显微外科治疗水平。方法 报告65例鞍结节脑膜瘤的临床表现，诊断和显微外科治疗结果。结果 所有病例均在手术显微镜下进行，肿瘤全切除率为93．8％，手术死亡率为3．1％。术中一侧或双侧嗅神经的保留率为86％。仔细辩认和分离垂体柄，防止损伤后发生尿崩症，垂体柄的保存率达95．4％。术中颈内动脉或其它血管破裂出血，可行显微缝合。大多数病人术后视力得到满意的恢复。结论 根据肿瘤的部位和大小，选择恰当的手术入路。应用显微外科技术能够显著提高肿瘤的全切率和降低手术死亡率。

Microsurgical treatment of tuberculum sellae meningiomas: Review of 6 5 cases

Objective To improve the microsurgical techniques for tuberculum sellae meningioma. Methods A series of 65 consecutive patients treated microsurgically were reviewed retrospectively. Results All patients underwent craniotomy with an operating microscope. Complete removal was performed in 61 cases (93 8%). Mortality was 3 1%. It was possible to preserve the olfactory tracts bilaterally or unilaterally in 86%. Most patients showed good visual improvement after operation. Conclusions Use of the operating microscope appeared to increase the total removal rate while lowing the mortality.
