| 摘 要： | 目的探讨肝功能Child-pugh分级与肝硬化患者食管曲张静脉套扎(EVL)术疗效及预后的相关性。方法分析213例行EVL术肝硬化患者的病历和随访资料,采用COX比例风险回归模型分析EVL术后影响患者生存的因素,Kaplan-Meier(K-M)法估算肝功能Child-pugh各级患者的生存率。结果共156例患者完成食管曲张静脉根除治疗。肝功能A、B、C级患者的曲张静脉复发率分别为15.2%、30.8%、53.3%,C级与A、B级比较,差异有统计学意义(P<0.05),A、B级间差异无统计学意义(P>0.05),肝功能A、B、C级患者的出血复发率分别为3.0%、5.1%、8.9%,三者间差异无统计学意义(P>0.05),肝功能A、B、C级患者的5年生存率分别为82.79%、54.94%、28.71%,且其生存曲线两两比较,差异有统计学意义(P<0.05)。结论肝功能Child-pugh分级与EVL术后曲张静脉的复发率和患者生存率密切相关,肝功能越差,曲张静脉复发率越高,患者的生存率越低。
